Registered Nurse (RN) – Utilization Review / Care Management (Part-Time, Day Shift)
Location: Anacortes, WA
Schedule: 24 hours per week | Day Shift
Salary Range: $39.57 – $59.35 per hour

Island Health is seeking an experienced Utilization Review RN to join our dedicated Care Management team. In this key role, you’ll collaborate with providers, patients, families, and payors to coordinate care, guide safe discharge planning, and ensure appropriate utilization of hospital services.
This position is ideal for a compassionate, detail-oriented nurse who thrives in a fast-paced environment and enjoys problem-solving, patient advocacy, and teamwork.
What You’ll Do
As part of our interdisciplinary Care Management Team, you will:
Discharge Planning
Assess and identify patients’ post-hospital needs to ensure safe, timely, and appropriate discharge plans
Coordinate referrals and communicate with community agencies, home health, rehab, and skilled nursing facilities
Educate patients and families about discharge plans, expectations, and available resources
Document assessments, interventions, and outcomes clearly and promptly
Partner with the interdisciplinary team to develop and communicate individualized care plans
Utilization Review
Perform prospective, concurrent, and retrospective medical reviews for medical necessity and appropriate level of care
Collaborate with the Physician Advisor and payors on coverage determinations
Conduct risk assessments for new admissions and identify those needing care management intervention
Apply InterQual or MCG guidelines to evaluate patient status and medical necessity
What You’ll Bring
Current WA State RN license (required)
Graduate of an accredited school of nursing; BSN preferred
MCG certification (required within 6 months of hire)
Minimum 3 years of hospital experience, preferably medical-surgical
Minimum 1 year of discharge planning, case management, or utilization review experience
Strong communication, collaboration, and critical-thinking skills
Knowledge of CMS regulations, utilization review, and payor requirements
Ability to manage multiple priorities in a fast-paced environment with professionalism and empathy
