We are seeking a compassionate, skilled medical provider who is passionate about caring for seniors. This role offers the independence of working autonomously while being supported by a collaborative, mission-driven team. Both full-time and part-time schedules are available.

Position Overview

We are looking for a Palliative Care Specialist (NP or PA) to provide compassionate, patient-centered care for individuals with chronic or serious illnesses. This is a mobile, in-person care role with the opportunity to work closely with an interdisciplinary team while maintaining autonomy in managing your caseload.

Key Responsibilities

  • Provide comprehensive palliative care consultations

  • Develop and manage individualized care plans focused on symptom relief and quality of life

  • Collaborate with physicians, nurses, social workers, and caregivers

  • Educate patients and families on illness progression and goals of care

  • Monitor outcomes and adjust care plans as needed

  • Participate in team meetings and case reviews

Qualifications

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Nevada (or eligibility to obtain)

  • 1–3+ years of experience in palliative care, hospice, or chronic disease management

  • Strong communication skills and a patient-first approach

  • Ability to work independently and manage a diverse caseload

  • Telehealth experience is a plus

  • Valid driver’s license, reliable vehicle, and current insurance
