Position Overview
We are looking for a Palliative Care Specialist (NP or PA) to provide compassionate, patient-centered care for individuals with chronic or serious illnesses. This is a mobile, in-person care role with the opportunity to work closely with an interdisciplinary team while maintaining autonomy in managing your caseload.
Key Responsibilities
Provide comprehensive palliative care consultations
Develop and manage individualized care plans focused on symptom relief and quality of life
Collaborate with physicians, nurses, social workers, and caregivers
Educate patients and families on illness progression and goals of care
Monitor outcomes and adjust care plans as needed
Participate in team meetings and case reviews
Qualifications
Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Nevada (or eligibility to obtain)
1–3+ years of experience in palliative care, hospice, or chronic disease management
Strong communication skills and a patient-first approach
Ability to work independently and manage a diverse caseload
Telehealth experience is a plus
Valid driver’s license, reliable vehicle, and current insurance
