Targeted Case Manager-
New Hope Services is looking for a Case Manager who has the primary responsibility for coordination client care, supervision of assigned cases, including assessment, support planning, referral and related activities, monitor and follow-up.
New Hope Service, located in Pittsburg and Valley Center, Kansas, provides developmentally disabled clients with the services and support they need to empower them to live independently within their community. Medicalodges has developed, in New Hope Service, the expertise and highly trained personnel that are necessary to help people with developmental disabilities move into residential settings and personal homes and away from institutions.
Job Responsibilities:
- Assessments such as individual and family interviews, completion of one or more assessment instruments, arranging for profession evaluation or assessments, compiling or completing referral or placement packets.
- Service Planning such as: facilitation of a person-centered support plan for services and supports, contacts (meetings, telephone calls) concerning an individual’s plan, reviewing and monitoring the plan and the individual’s progress, providing or arranging for quality of services provided.
- Assessment and development of Behavior Modification Plans and Support Profiles.
- Service Coordination such as providing and/or sharing information about services and resources, referral for services, follow-up on referrals, participating in transition planning, oversight of services, evaluating the individual’s satisfaction with supports and services and linking the individual to community resources, supports and services.
- Community Inclusion such as: Researching available services, contacting and integrated supports services, developing and accessing natural supports and services such as volunteers, neighbors, colleagues and family and time limited independent living training of the individual.
- Personal assistance such as: securing and maintaining SSI, SSA or SSDI, HUD, Medicaid/Medicare, establishing special plans for work, assisting in representing the individual at meetings and hearings, etc.
- Transportation of the individual when a specialized knowledge and coordination is necessary for obtaining and communicating the results of evaluations, generic care, benefits or supports and services relative to the individual’s plan of services. Reasonable transportation to meet with the individual.
- Communication with the individual, designated representatives, support or service providers or other relative to the service plan and documentation of services provided.
Job Requirements:
- A minimum of 6 months experience in the field of developmental disabilities;
- A bachelor’s degree or 48 additional months of relevant experience (i.e., 6 months of experience to substitute for each semester of college education)
