Transition Services Specialist - $22.00/hr.

General Description
We are seeking a passionate and dedicated Transition Specialist to support special education students and young adults (17-21 years old) with developmental disabilities in developing social, intellectual, and vocational skills. In this role, you will foster person-centered, community-based learning and employment opportunities through collaboration, coaching, and career planning.
Your work will make a direct impact by guiding individuals through their transition from educational settings into the workforce-helping them discover their strengths, build essential job skills, and connect with meaningful employment.
Essential Functions
- Conduct person-centered assessments using interviews, observations, and career research.
- Develop and maintain partnerships with community businesses for work experience placements.
- Coordinate and coach students through job shadowing, work experiences, and career exploration.
- Remain present with students during all support shifts.
- Facilitate interactions between students and employees to build workplace social skills.
- Provide training in job readiness skills: communication, problem-solving, workplace etiquette, etc.
- Support students in identifying core competencies and developing individualized career plans.
- Deliver job development services including resume support, interview prep, and employer outreach.
- Provide direct job coaching and in-the-moment support on-site.
- Maintain high-quality documentation for school and ORS service reporting.
Knowledge, Skills & Abilities
Strong knowledge of employment readiness, job coaching, and accommodations.
Excellent interpersonal and communication skills with students, families, schools, and employers.
Experience in goal-oriented, one-on-one employment support.
Strong organizational skills and ability to work independently and as part of a team.
Ability to adapt, take initiative, and maintain accurate documentation.
Qualifications
- Required: High School Diploma or GED
- Preferred: Associate's or Bachelor's Degree in Special Education, Psychology, or related field
- Preferred: ACRE Certification or equivalent
- Experience: Minimum 1 year supporting individuals with intellectual and developmental disabilities
- Other Requirements: Valid driver's license, vehicle inspection, registration, and insurance
Compensation & Benefits
Hourly Pay: $22.00
