Starting Pay Range - $23.53-$25.87 /hr

Summary:

This position is highly community based. The primary focus of this position will be a housing first model and will focus on the Forensic populations with referrals originating from multiple sources. Duties include working on site at the jail. The Housing Case Manager will develop both consumer and landlord relationships, assist consumers in obtaining and maintaining housing and an independent living status, and assist with all housing resources. The Housing Case Manager will be responsible for completing intakes, housing plans, charting in an EMR and submitting both monthly and quarterly information required for tracking. 

Essential Job Responsibilities:

  • Familiar with Landlord Tenant Law and ensure that CRMHS is in compliance with applicable laws for housing programs.
  • Familiar with location matching, roommate matching, background checks, leasing/application packets
  • Familiarity with Rent/Lease information and assistance (e.g. notices, evictions, housing support, rent collection)
  • Coordinate with community partners and organizations for referrals, housing placements, braided funding and long term supports 
  • Working with clients while in temporary housing, including shelters or hotels.
  • Facilitate consumer groups as needed
  • Provide all aspects of Housing Support Services to individuals enrolled
  • Act as Liaison between consumer, landlord, other partnering agencies
  • Coordinate with local profit and non-profits (e.g. HEN, VHA, Home and Community Services, Clark County Jail, SUD, Crisis, etc. Must be able to gain jail access. 
  • Participate in weekly and monthly meetings as applicable
  • Work as a team member with other CRMHS staff and other agencies when appropriate
  • Coordinate care internally to assure timely communication relative to consumer needs and preferences
  • Maintain all appropriate records and charts

Non-essential Job Functions:

  • Attend staff and planning meetings as scheduled.
  • Maintain communication and cooperative working relationships within the agency.
  • Other duties as assigned by Supervisor.

Education, Licensing and Experience:

  • Approximately 2+ years of experience in the social service field with mental illness, medically fragile and/or chemical dependency and housing preferred.  
  • Minimum of Bachelor’s degree (or equivalent work experience) preferred.

Competencies, Knowledge, Skills and Abilities:

  • Good communication and interpersonal skills, basic knowledge of chronic mental illness, customer service, maintaining and building therapeutic relationships.  
  • Assessment skills, multi-tasking, making decisions, problem solving, and good time management skills.  
  • Flexible and open to change.
