Hospice of Marion County, a member of Empath Health is currently seeking a Care Navigation RN Facilitator to join our team.
Monday – Friday 8am to 5pm.
The role of the Care Navigation Nurse Facilitator is to supervise and lead a team of Care Navigators by providing oversight and coordination of their day-to-day work, assessing performance, and providing feedback to drive results. In addition, this individual will work directly with community partners outside of the Empath Health service area to ensure smooth facilitation of patient transfers/travelers to our service area and optimal access to the Empath Health Integrated Network of Care.
JOB DUTIES/RESPONSIBILITIES:
The Care Navigation Facilitator is responsible for performing all of the job duties of the Admissions RN as well as those noted below.
- Promotes, articulates and consistently models the vision, mission and values of the organization.
- Collaborates with out of area community partners regarding patient care needs and providing the “right care at the right time” to insure optimal access.
- Contributes to the development of a highly functioning team.
- Actively participates in ongoing review of Care Navigation procedures.
- Continually reinforces the Empath Health infrastructure to support rapid, appropriate responses to patient/family and community needs/service requests. Continually builds resources to educate, empower and support patients and families.
- Consistently updates knowledge of Empath Health Care Continuum. Participates in educational offerings to maintain or update skills.
- Works directly with other members of the department to facilitate effective, prompt and efficient response to all inquiries.
- Performs a self-evaluation of strengths and weaknesses yearly and develops annual self-development plan.
- Gathers information related to patient’s present condition/status, plan of care, discharge or travel plans.
- Inputs and coordinates new Inquiries/Referrals into the electronic medical record.
- Provides introduction, education and support to patients, family and care partners regarding decision making.
- Advocates for and oversees discharge planning to and from different care settings.
- Communicate with internal coworkers including Empath Health physicians regarding patient’s eligibility for hospice care.
- Obtains and documents verbal certification of patient’s prognosis depending on Care Program.
- Assesses the need for any resources outside the Empath Health Integrated Network of Care.
- Communicates and documents according to agency practices and guidelines.
- Works directly with Care Navigation, Admission nurses, Leadership, Liaisons and IDT staff to provide thorough transfer and admission experience for those we serve.
- Works directly with other departments within Empath Health including but not limited to Finance, Reimbursement, Organizational Excellence and IDT’s to ensure a thorough transition between hospices for any Transfer patients coming to our service area.
- Promotes positive rapport and presentation in all interactions.
- Openly communicates patient/family needs to all others involved in the patient’s care in accordance with Empath Health Communication Practices.
- Keeps compassion and empathy at utmost priority.
- Seizes every opportunity to interact with customers to better understand their perspectives, expectations and concerns regarding care needs so that services can be refined to align with their expectations.
- Completes documentation within electronic medical records in accordance with Empath Health policies, processes, guidelines and HIPAA guidelines.
- Completes referral processing and admissions in any care setting depending on need.
- Participates in Admission RN on-call rotation.
- Performs other duties as specified by Care Navigation/Admissions/Business Development Leadership.
REQUIREMENTS:
- Education: Associate’s degree (A.A.) or equivalent from two-year college or technical school; bachelor’s degree preferred
- Valid Florida license as a RN or LPN
- Strong preference for minimum two (2) years of hospice experience as RN or LPN
- Valid Florida State Driver’s license
- Continuing education: As required for licensure
- Knowledge of hospice regulations such as admission/qualification criteria, reimbursement, and levels of care preferred.
- Excellent, positive relationship building, outreach efforts, problem solving, organizational, time management, creativity, interpersonal skills, written and verbal communication a must.
- Strong computer skills, including knowledge of Microsoft Office applications and Electronic Medical Record
