SUMMARY: Analyze and evaluate health records to ensure completeness and accuracy and compile statistics. Maintain Health Information filing system in accordance with policy and procedures. Maintain and control release of patient information to authorized persons and may participate in the compilation and documentation of provider credentialing.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Function as a part of the management team; work closely with physicians and abide by and enforce all policies.
  • Work closely with Administrator informing of any patient related issues and the coordination of plans to resolve the issues.
  • Managing scheduling and supervision of daily duties of health information staff.
  • Ensures the department is in compliance with the rules and regulations of HIPAA. Supervise and monitor staff performance and evaluate personnel using organizational guidelines and departmental goals and objectives.
  • Assigns ICD-9CM coding of inpatient, outpatients, swing bed, ER and Physical Therapy. 
  • Ensures CPT codes are assigned to procedures.
  • Ensure accuracy, consistency and uniformity of data recorded in the medical records.
  • Manage the dictation process.
  • Assist with transcription as needed.
  • Provide copies of medical records to third party requests in accordance with the established policies and procedures of the facility.
  • Responsible for preparation of medical staff meetings and recording the minutes.
  • Ensure completion of charts. 
  • Responsible for the implementation of and compliance with all safety policies and procedures within the department.
  • Submit census reports in a timely manner.
  • Attend continuing education classes as requested and required.



Requirements

QUALIFICATIONS:

  • Knowledge of ICD coding.
  • Must possess ability to become competent with the medical billing system.
  • Competent in medical terminology.
  • Knowledgeable in lab and radiology procedures.
  • Competent in Microsoft Office Word and Excel.
  • Maintain discretion and confidentiality in communications.
  • Familiar with policies and procedures regarding reporting and release of protected health information (PHI).

EDUCATION and/or EXPERIENCE:

  • High School Diploma/GED
  • Certified coding specialist preferred
  • Registered Health Information Technology Certification preferred
  • 2 to 4 years experience preferred

LANGUAGE/READING SKILLS:

  • Must be able to communicate adequately and effectively with patients served by the hospital regarding health histories.

MATHEMATICAL SKILLS:

  • Must be proficient at mathematical and statistical calculations.

REASONING ABILITY: Employee responds to unusual or varied situations that are not covered by existing standards, procedures and precedents. 


WORK ENVIRONMENT: Employee is regularly required to stand, walk, sit, ascend and descend stairs, possess ability to handle, finger or feel objects, tools or controls; reach with hands and arms. 

Employee must frequently be able to lift 25 pounds from the floor to waist level. Specific vision abilities include close vision and the ability to clearly focus vision.
