Do you have a heart for guiding youth and families toward stability and success? As a Case Management Facilitator in the Intensive Case Management (ICM) program, you’ll play a key role in bringing people together to strengthen resilience and build lasting support systems across Sonoma County.
The Case Management Facilitator provides family-centered mental health counseling, support, crisis intervention, and case management services for youth and their families. Facilitators are members of a multi-disciplinary team and provide trauma-informed, culturally responsive services in the community at times and locations convenient to the family. Facilitators create safe spaces for collaboration, help families and youth navigate challenges, and foster connections to resources and networks that support long-term wellness and growth.
ABOUT Intensive Case Management (ICM)
The ICM Program is a short-term, community-based service for youth on probation and in the Sonoma County Collaborative Court. The program connects youth and families with needed resources to reduce justice system involvement, stabilize youth in the community, and strengthen both formal and informal support networks. Grounded in Seneca’s commitment to safe, nurturing, and respectful care, ICM helps families build confidence, coping skills, and resilience to manage behavioral or emotional challenges. The ultimate goal is to equip youth and caregivers with the skills to thrive at home and prevent entry or re-entry into congregate care.

RESPONSIBILITIES
- Conduct assessments to identify needs, barriers, and strengths to establish individualized goals and action plans with clients, caregivers, and school teams
- Provide counseling and case management services for enrolled youth and their families
- Support client and family to connect to natural supports and community-based resources
- Provide crisis intervention and other mental health services as needed at client homes, schools, and other community locations
- Participate in all treatment reviews, program staff meetings, and Individualized Education Program (IEP) meetings when necessary
- Complete mental health notes and other paperwork, and complete service tracking documentation as indicated
- Assist with planning arrangements and transportation for school, therapy appointments, and court hearings as neede
- Attend team meetings, weekly trainings, individual and group supervisions, and adhere to clinical best practices
- Maintain a flexible work schedule
- Participate in providing emergency on-call response as scheduled
QUALIFICATIONS
REQUIRED
- Education/Experience Requirements:
- Bachelor’s degree OR
- Associate degree or equivalent + 1 year of applicable experience OR
- High School Diploma/GED + 2 years of applicable experience
- Applicable experience can include paid or unpaid experience working with youth or families or working in the social services field
- Flexible schedule with the ability to work some evening hours as needed
- Participate in providing emergency on-call response as scheduled
- Driving within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region.
- Must be at least 21 years of age
- Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy
- TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
PREFERRED
- Bilingual Spanish skill set
- Experience working with children in out-of-home care
- Ability to work as a team player, with diverse population and demonstrates cultural competency
SCHEDULE
- Full-time; Monday - Friday, 9am - 5:30pm
- Hybrid; provide in-person services & complete documentation remotely

- Starting at $24.04 - $26.54 per hour, commensurate with experience
- Additional compensation provided upon passing bilingual language proficiency exam
