Physical Therapist – Travel Position

Job Description

We are seeking a dedicated Physical Therapist to join a healthcare facility in Cortez, CO, on a travel contract. The Physical Therapist will evaluate patients, design individualized treatment plans, and deliver therapy to restore mobility, strength, and independence. The ideal candidate is compassionate, adaptable, and skilled in providing patient-centered rehabilitation services.

Key Responsibilities

• Perform comprehensive evaluations of patients’ mobility, strength, and therapy needs.
• Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary teams.
• Provide therapy interventions such as gait training, strengthening, balance activities, and functional mobility exercises.
• Educate patients and caregivers on therapy goals, safe mobility techniques, and home exercise programs.
• Document evaluations, progress notes, and therapy sessions accurately and in compliance with facility and regulatory standards.
• Collaborate with occupational therapy, speech therapy, and nursing staff to ensure comprehensive patient care.
• Support patients in regaining independence and improving quality of life through evidence-based practices.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in Colorado (or compact license).
• Minimum of 1 year of recent PT experience, preferably in rehabilitation or acute care settings.
• Current certification in Basic Life Support (BLS).
• COVID-19 vaccine and recent tuberculosis test.
• Strong clinical assessment, documentation, and communication skills.
• Compassionate, patient-centered approach with adaptability to diverse patient populations.
