Join our team of case managers responsible for assisting residents with accessing benefits, health and wellness activities and additional services to stabilize their housing within formerly managed NYCHA housing who are under the Permanent Affordability Commitment Together (PACT) initiative.  The PACT initiative is part of the Next Generation NYCHA strategic plan, preserving the affordability of residents’ homes while accessing new resources to make long-overdue repairs.  Join us and work to improve the overall financial and health of residents to ensure they remain housed within vibrant communities. 


RESPONSIBILITIES 

  • Assist with the outreach and engagement of community members in the Sack Wern PACT-RAD development.
  • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up.
  • Manage a case load.
  • Monitor and document client progress toward service plan goals.
  • Document all interactions with, or on behalf of, clients.
  • Maintain familiarity with program resources available on- and off-site.
  • Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers.
  • Complete program reports on a monthly basis, or more frequently as required.
  • Act as program contact person for inquiries concerning clients.
  • Provide collaboration and support with securing benefits, especially as it relates to access to benefits services and eviction prevention efforts.
  • Report to, and meet with, supervisor on a monthly basis.
  • Conduct home visits and other field work, as required.
  • Coordinate and facilitate monthly activities that fit the needs of the PACT-RAD community.
  • Perform additional administrative duties as assigned.
  • Assist with community events as needed and serve as a liaison with community vendor.

 QUALIFICATIONS 

  • Bachelor’s degree with case management experience.
  • Experience using Access HRA and securing HRA entitlements and benefits preferred.
  • Mental health background preferred.
  • Crisis prevention and mental health first aid training is required within first 6 month.
  • Strong oral and written communication, time management and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.  

 PHYSICAL REQUIREMENTS 

  • Ability to use a computer for prolonged periods. 
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time. 
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents. 
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.  
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
