Registered Nurse (RN) Case Manager – Summit County

We are currently seeking a Registered Nurse (RN) Case Manager to join our dedicated team and help patients remain safe and healthy in the comfort of their homes in Summit County.

Qualifications

We are looking for a compassionate and dependable RN who:

• Holds an active RN license in Ohio with at least 1 year of experience. (Home health or geriatric experience is a plus, but not required.)
• Communicates effectively and professionally with patients, families, and interdisciplinary team members.
• Demonstrates strong organizational skills and the ability to manage a flexible schedule independently.
• Values teamwork and is dedicated to delivering high-quality patient care.
