    Hourly Rate: Level I: $25.23 / Level II: $28.22 / Level III: $33.54

    PRINCIPAL DUTIES AND RESPONSIBILITES:

    • Explain the range of Howard Brown Health’s medical and psychosocial services to Ryan White Part A clients.
    • Provide information, education, and emotional support to patients/clients in a compassionate, culturally sensitive, and appropriate manner.
    • Maintain an active caseload of 30-45 medical case management clients.
    • Maintain, at minimum, monthly contact with every client on assigned caseload.
    • Complete an initial assessment with newly referred adult HIV+ individuals requesting medical case management services at Howard Brown Health and/or as referred by the AIDS Foundation of Chicago and conduct subsequent required reassessments every six months.
    • Develop initial and continuing care plans with enrolled clients to assist in identifying needs and to provide coordination of clinical and social services.
    • Provide education, information, and referrals to clients; locate, identify, and utilize new social services and resources.
    • Evaluate all clients’ abilities to understand health plans; provide treatment education and adherence counseling to clients to improve health outcomes; and consult with medical providers regarding assessment and counseling sessions.
    • Coordinate medical care with providers, schedule patient follow-up (internal and external) appointments; facilitate appointments with collaborative medical partners for provider recommended follow-up screenings and treatment.
    • Consult and collaborate with a multi-disciplinary team including primary care providers, behavioral health staff, nursing, MAs, other members of client care teams, partner services staff, and other Howard Brown staff involved in client/patient care.
    • Assess financial need of clients and facilitate patient financial support, including help with applications, especially as it relates to public benefits programs, health insurance (Medicaid/ACA Marketplace), ADAP, and prescription assistance programs.
    • Provide referrals on an ongoing basis of relevant services, including SNAP, ADAP, housing, vision, DHS, SSA, unemployment, insurance, and prescription assistance.
    • Develop and maintain required records, reports, and statistical data. This includes maintenance of caseload, completing activity tracking sheet, progress notes, resource receipts, and tracking health maintenance of each client.
    • Follow appropriate standards of care and support clients with tracking and completing necessary health and wellness screenings including, but not limited to, standard HIV labs, TB testing, vaccines, dental exams, STI testing, and sexual and reproductive health screenings.
    • Assist with arranging for patient transportation as needed.
    • Attend and participate in all group consultation/case conferences, social service department meetings and trainings, team meetings, individual supervision, and any other required staff meetings and trainings.
    • Provide brief case management services on an as needed basis through a “Case Manager on Duty” on-call system.
    • Assist in the supervision and training of volunteers and interns who provide services to clients as needed.
    • Maintain the highest level of client confidentiality, in person and in practice.
    • Travel among agency locations based on patient need.
    • Other duties as assigned.

     

    Additional Job Characteristics:

     

    WORKING CONDITIONS:

    Primary work setting is an indoor office/health care environment.  Position will involve some travel and staffing at multiple sites where clinics are held. 

     

    QUALIFICATIONS, SKILLS AND ABILITIES:

    Required:

    • A Master’s/Bachelor’s degree in a human services field; or a registered nurse; or a Master’s/Bachelor’s degree in non-human services field and 2 years case management experience; or a 2-year Associate’s degree with 4 years case management experience
    • Working knowledge of the health-related disparities faced by the LGBTQ population.
    • Demonstrated interpersonal skills and ability to engage and work effectively with a diverse staff and patient population required.
    • Bilingual-English/Spanish 
    • Writes in a concise and organized manner and uses correct grammar.

     

    Preferred:

    • Knowledge and experience navigating health and medication coverage systems, highly regarded
    • Prior knowledge and experience working with patients living with HIV and with diverse communities
    • Experience working in a community based medical/social services setting
    • Knowledge and experience with working with clients living with HIV/AIDS within diverse communities
    • Experience working with a high volume of clients in a fast-paced environment

    • Requires ability to speak audibly and listen actively.

    • Requires ability to sit for extended periods of time.

    • May require periodic travel among Howard Brown Health locations based on patient need.
