Description

Park Place of Elmhurst, a nonprofit five-star CCRC living community, is located across from Endeavor Health Elmhurst Hospital. Our 16-acre arboretum-designated campus offers 180+ customizable independent living apartments and a full continuum of care, including assisted living, short-term rehabilitation, skilled nursing, and memory support. We are seeking an Admissions Director/Clinical Assessment Coordinator to join our efforts in providing the most cohesive admissions experience for our CMS 5-Star rated and Joint Commission Accredited 37-bed skilled rehabilitation center.


Description

We are seeking a full-time Admissions Director/CAC to guide residents and families through the admission process and to maintain and grow census by developing, marketing, establishing a relationship and networking with referral sources. Additionally, this person will be responsible for managing leads in the primary referral hospitals within Chicagoland (Endeavor, Northwestern, Rush, Loyola, Gottlieb, MacNeal, Shirley Ryan, Advocate) ensuring a smooth admissions process for patients transitioning from a hospital setting to our skilled nursing facility. The ideal candidate will also identify new opportunities for partnering and business growth. The Admissions Director/Clinical Assessment Coordinator must have knowledge of Medicare & Managed Care guidelines and clinical understanding and competence related to the referral process.

Requirements

  • Bachelor’s Degree from an accredited college/university or equivalent preferred. 
  • Must possess two years sales experience, preferably in health care services or direct experience as an Admission’s Director/Coordinator in Skilled Nursing.
  • Must have a thorough knowledge of Medicare and Commercial Insurances.
  • Must have an understanding of clinical conditions and common medications and must be adept at assessing patients for admission to Park Place. 
  • Must possess a thorough knowledge of principles of effective communication, policies and education, community relations, demonstration, organization structure, social and activity services, government relations etc., as they relate to senior living.
  • Must possess the ability to communicate effectively, orally, electronically, and in writing.
  • Must possess the ability to assimilate information from a variety of sources, analyze information and make recommendations to the Administrator.
  • Ability to travel locally.
  • Must have excellent organizational skills.
  • Must have patience, tact, a cheerful disposition and enthusiasm.
