The Wraparound Facilitator’s primary responsibility is to facilitate Child and Family Planning Meetings using Wraparound principles that are family centered, individualized, strengths based, and outcome focused. The program’s goal is to help children and families with complex and enduring needs receive intensive treatment in the least restrictive environment that is appropriate. The Wraparound Facilitator’s role is to provide assessment of the client and family’s needs and lead the team in providing treatment to support the family toward meeting their goals.

About Wraparound

Wraparound is a family planning process that provides therapeutic support to youth and families in the community with the intention of enhancing natural resources and keeping youth in the least restrictive and most family-like setting, with the intention of reducing the need for institutional care. Services range up to 12 months and include regular Child and Family Team Meetings that support youth and families with identifying strengths, creating realistic goals, and developing a comprehensive step-by-step plan based on those goals. Youth and families are referred directly by Contra Costa County (CCC) Children and Family Services, the CCC Juvenile Probation Department, CCC Children’s Behavioral Health, or Kaiser.

Responsibilities:

  • Facilitation of Child and Family Planning Meetings. This includes meeting preparation, coordinating attendance, facilitating the meeting, writing meeting notes and distributing them within two days, writing mental health notes, and cleaning up after the meeting.
  • Case management and coordination of care.
  • Collaborate with county and community partners including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc.
  • Complete mental health notes, Family Team Meeting notes, Wrap Action Plan, incident reports, and other documentation as necessary in a thorough, accurate, and timely manner.
  • Intervene with clients’ behavior management programs and provide crisis management as needed.
  • In circumstances where there is serious threat of harm, this may include physical management/restraint of the clients (implementation of Seneca’s Equilibrium model).
  • Attend team meetings, weekly trainings, individual supervision, and group supervision.
  • Adhere to clinical best practices.
  • Complete clinical documentation appropriate to the MHRS credential, including but not limited to assessments, treatment planning, etc.
  • Maintain a flexible schedule that includes early mornings, evenings, and occasional weekends as needed.
  • Participate in providing emergency on-call response as scheduled.
  • Function as a program leader and uphold the ideals of the program.

Qualifications:

Required

  • Education + experience requirements:
    • Bachelor's Degree + 4 years experience working with children/youth and families with complex and enduring needs in a mental health setting
    • Associate's Degree + 6 years experience working with children/youth and families with complex and enduring needs in a mental health setting
  • At least 21 years of age.
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy.
  • Own vehicle and current automobile insurance.

Preferred

  • Experience implementing family-focused wraparound services

Skills of an Ideal Candidate

  • Ability to effectively work with a diverse population and workforce.
  • Strong organizational, time management, and writing skills.
  • Ability to work effectively as part of a multidisciplinary team.

Schedule

  • Full-time, Monday - Friday.
    • Flexibility with schedule is required to accommodate schedules of clients and families.
  • 3-4 on call shifts a month.

  • Starting at $34.83 - $37.33 per hour.
    • Actual salary dependent on creditable experience above the minimum qualifications for the role.
