Description
The Emergency Shelter Case Manager is responsible for client intervention and engagement upon their entrance into the Emergency Shelter. This includes new intakes and assessments, service planning, case management services, advocacy and counseling. The Emergency Shelter Case Manager, along with other Shelter staff will address the basic needs of clients. They will provide supportive services as needed to address the tailored needs of individuals and families. The Emergency Shelter Case Manager is also expected to identify and outline opportunities and pathways out of the Emergency Shelter.
They will collaborate with staff and volunteers and assist the Shelter Manager with the day-to-day operations. They will document client service transactions in Salesforce, HMIS and other databases as required and perform assigned operational and administrative tasks as necessary to fulfill program objectives.
JOB RESPONSIBILITIES:
Responsibilities
- Complete intake and assessment including the completion of all necessary Housing Forward and HMIS forms to ensure proper registration of all new clients.
- Provide learning opportunities and resources for individuals and groups.
- Provide clients with an orientation to the Emergency Shelter services and rules.
Case Management Responsibilities
- Provide case management services including advocacy, crisis intervention and counseling to program participants at the Emergency Shelter and external settings which may include public libraries, resource centers and other Housing Forward sites.
- Orient clients to the full range of public programs and housing support while focusing clients on individual service plans to guide their transition out of the Emergency Shelter.
- Develop one-on-one and group-oriented information sessions and learning opportunities which help clients in establishing goals toward personal wellness, self-sufficiency and viable housing solutions.
- Assist clients in obtaining essential documents of Real IDs, birth certificates, social security cards, voter registrations, passports and credit reports in addition to developing financial goals, personal financial budgets and time management plans.
- Make referrals in accordance with service plans to internal programs and external providers. Initiate follow-up of referrals to coordinate services and monitor client progress.
- Ensure that clients have completed all necessary documentation to be placed on housing lists.
Administrative
- Complete reports of unusual incidents as necessary and submit to supervisor for review.
- Maintain accurate and confidential records (e.g. case notes) in accordance with agency policies. Responsible for accurate and timely data entry of service transactions into HMIS (Homeless Management Information System).
- Assist with the preparation of aggregate data reports and statistics.
Other Duties
- Assist in the collaboration of volunteers.
- Attend and participate in staff meetings.
- Attend agency wide and departmental meetings.
- Participate in clinical case staffing.
- Other duties as assigned.
Requirements
SCHEDULE:
This is a full-time, exempt position. The Emergency Shelter Case Manager will be required to work some evenings, Monday, Wednesday, Friday 12:00pm to 8:00pm to accommodate shelter hours, must be flexible.
QUALIFICATIONS:
Bachelor’s degree in Social Work, Master’s preferred, or related field or minimum of two years relevant experience.
KEY COMPETENCIES:
- Skills or experience in the following areas: Homelessness issues, mental health, substance abuse, advocacy, motivation and goal setting and planning.
- Recognizing and accessing consistent, reliable supportive services.
- Strong written and oral communication skills.
- Must be able to manage multiple tasks
- Solid knowledge of computer programs including Microsoft Word and Excel Must have smartphone to utilize Agency communication and collaboration tools.
- Valid driver's license and proof of insurance

SALARY: $50,000 - $53,000
