The Role You’ll Play

As a Primary Care Coordinator, you will be the vital link between patients and their care team. You’ll support our Family Nurse Practitioner (FNP), Registered Nurse (RN), and other healthcare professionals to ensure every patient receives seamless, high-quality, and integrated care. This role is about more than scheduling appointments—it’s about building relationships, helping patients navigate the healthcare system, and ensuring their physical and behavioral health needs are met with compassion and efficiency.


The Day-to-Day:

  • Serve as the main point of contact for patients, making sure they understand their care plans and follow-up steps
  • Schedule and track appointments, referrals, and diagnostic tests to keep patients on track with their treatment
  • Work closely with the care team to coordinate services for patients with complex medical and behavioral health conditions
  • Monitor patient progress through follow-up calls, education, and reminders to support medication adherence and lifestyle changes
  • Advocate for patients by connecting them to community resources and addressing barriers such as transportation or financial constraints
  • Maintain accurate, timely documentation in the electronic health record (EHR) system
  • Collaborate with colleagues in team meetings, ensuring patient care plans are current and effective

Requirements

Qualifications:


  • High school diploma or equivalent required; additional education or certification as an LPN, CMA, or similar credential preferred.
  • Experience in healthcare coordination, case management, or patient support, preferably in a primary care or behavioral health setting.
  • Knowledge of healthcare systems, community resources, and the ability to assist patients with navigating complex care needs.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work collaboratively within an interdisciplinary team.
  • Proficiency in using electronic health records (EHR) and other healthcare management tools.
  • Understanding of patient confidentiality, HIPAA regulations, and healthcare documentation standards.
  • Bilingual skills a plus (if applicable to the patient population served).
