Registered Home Health Aide - Hospice bei VNA (Visiting Nurse Association)

Job Summary: Our Home Health Aides provide direct patient care under the supervision of a Registered Nurse or other qualified health professional team member in accordance with the plan of treatment or care plan. 

Key Responsibilities: 

• Provide personal care, including bathing hygiene, and dressing assistance
• Assist patients with mobility, including getting in and out of bed, walking and performing prescribed exercises
• Monitor and record vital signs such as blood pressure, pulse, temperature, respiration, weight, and pain levels  
• Accurately document observations, findings, and care provided in accordance with established charting standards
• work collaboratively with the care team to ensure continuity and quality of care

Education/ Experience/ Skills/ Qualifications: 

• Current Home Health Aide certificate from the State of California 
• CPR certification  
• Previous experience in home health, hospice, or a similar care setting preferred
•  Bi-Lingual Spanish helpful
