Full Time HUD Service Coordinator

Summary: Supports the Mission, Philosophies and Goals of the Agency. Coordinates service delivery for caseload by performing the following duties. This grant-funded position coordinates supportive services for low-income elderly persons and persons with disabilities so they may continue living independently in their communities.  

Education/Experience/Certificates/Licenses:  Bachelor's degree (B. A.) from four-year college or university or comparable related experience in working in a social services field preferred; Knowledge of Excel Spreadsheet software and Microsoft Word Processing software. Valid Kansas driver’s license. Must be able to travel 75% of the time (Coffee, Osage, Franklin).

Essential Duties and Responsibilities:

  • Must obtain Certification according to the HUD SC Grant guidelines.
  • Ensure service recipients apply for all entitled public benefit programs, thus reducing living expenses and avoiding eviction actions
  • Maintain public benefits spreadsheets. Supporting direct services with monitoring assets and other qualifications for such benefits. At random, checks fixed assets
  • Provide resources and support to property management staff regarding service coordination, including assisting individuals with completing initial application and recertification for HUD housing benefits.
  • Cultivate and implement service contacts with various community agencies serving elderly individuals with low incomes as well as persons with disabilities
  • Create a directory of community providers who support low-income elderly persons and persons with disabilities. Include agencies which provide health, legal, mental health, substance abuse, tax, and transportation services.
  • Work with local or chapters of national community health organizations to arrange for periodic mobile health screening events open to residents and the neighboring community 
  • Participate in or create networking meetings with other HUD-funded Service Coordinators and directors of programs serving the low-income aging population. Include agencies which provide health, legal, mental health, substance abuse, tax, and transportation services
  • Document activities engaged in by time and client or other purpose such as training. Additional documentation as necessary for persons receiving services. involvement. Monitor programs and supports to ensure implementation.
  • Must complete trainings as required within specified timeframe. (See Employee Handbook Section 2.11) Complies with all agency policies and procedures including HIPAA/Confidentiality policies.
