I.Summary of Position
An employee in this position coordinates and manages the day-to-day operation of the Day Habilitation program (Open Vistas). This includes the free-standing day habilitation site, individual day habilitation cases and day habilitation without walls programs. The Day Habilitation Coordinator shall oversee the development and growth of the programs with an emphasis on quality and diversity of individualized activities and experiences and shall be responsible for the safety and wellbeing of all the people we serve while in attendance at the program or in the community.
There is a direct relationship of the success and growth of the program to the variety and creativeness of the program activities developed. The Day Habilitation Coordinator will embrace the forward thinking ideologies of Adapt of the Hudson Valley?and OPWDD in the movement of individuals from more restrictive and isolative environments to those which foster free thought, individuality and independence. ALL plans developed by the Coordinator of Day Habilitation will embody these tenets.
II.?ORGANIZATIONAL RELATIONSHIP
The Day Habilitation Program Coordinator oversees two Assistant Coordinators, two shift leaders and 18 Direct Support Professionals (DSP). These staff work on-site and off-site in “without walls” community integrated programs.
II.REQUIRED DUTIES
1.Shall work with the individuals’ therapeutic team in the development of habilitation plans, behavior plans and safeguards.
2. Shall act as QIDP in the function of approving and certifying habilitation plans.
3. Shall be sensitive to and respect the individual rights, physical and emotional needs of each person and recognize the importance of communicating these needs both verbally and in writing to the appropriate support staff.’
4. Shall maintain strict fire safety protocols and maintain the fire plan performing regular fire drills in accordance with state regulations.
5. Shall monitor billing on a daily basis and ensure required documentation/notes are done, on-time and with quality.
6. Shall be responsible for fiscal management and timely reporting to the Finance Department. Will monitor and reconcile petty cash funds and funds of people receiving supports.
7. Shall monitor and maintain proper staffing levels.
8. Shall supervise staff and evaluate staff performance, recommend advancement, disciplinary action and/or termination to the Director of Community Services.
9. Shall hold regular, scheduled monthly meetings with all staff; will maintain a record of these meetings with staff signatures of attendance.
10. Shall work closely with the Assistant Coordinators of Day Habilitation in researching and locating resources in the community.
11. Shall successfully complete the required Medication Administration Certification course, and oversee the administering of medication according to physician's orders. Will also work with the Registered Nurse/Licensed Practical Nurse in monitoring all aspects of medication administration and documentation.
12. Shall ensure all mandatory agency in-service and annual trainings are scheduled for self and all staff.
13. Shall attend all required meetings; administrative, ISP reviews, roundtable meetings and agency
functions as required.
14. Shall serve as a liaison (ensuring open lines of communication) between staff, Medicaid Service Coordinators, Residential Staff, Management, Family Advocate, Quality Assurance staff, families and members of the community.
15. Shall transport individuals served in agency vehicles as needed, requested, or scheduled. Shall adheres to all agency, local, state and federal transportation guidelines/policies to ensure safety of individuals being transported.
16. Shall oversee the use of agency vehicles, maintenance and all required documentation.
17. Perform other duties as assigned or directed.
Essential Skills and Experience:
1. Have the ability to speak, read, and write English fluently which allows full communication with people served; Adapt of the Hudson Valley staff; and, 3rd parties.
2. Have the ability to relate appropriately to individuals with developmental disabilities.
3. Have a four year degree or equivalent in a related field eligible for QIDP status.
4. Have a minimum of two years’ experience working with people who have disabilities, preferably in a program or community setting
5. Experience / familiarity with electronic documenting databases such as Medisked
6. Possess computer proficiency, particularly with Microsoft Office applications and the ability to learn to use Precision Care and other database systems.
7. Must have prior supervisory experience and positive interpersonal relationship building skills
8. Have the ability to use sound judgment while following Adapt HV policies, practices and procedures.
9. Have the ability to operate office equipment (including computers).
10. Possess a valid driver license with a driving record acceptable to Adapt of the Hudson Valley's?insurance carrier and possess the ability to travel to all Adapt HV sites throughout the areas we serve.
11. Successfully meet all screening requirements (both prior to hire and during employment) as required by applicable regulation.
Nonessential Skills and Experience:
1. Possess knowledge of and experience with American Sign Language to communicate effectively in the language of the deaf.
2. Have a working knowledge of the community and willingness to cultivate and partner with community entities such as businesses, civic groups, and national organizations.
3. Prior work experience using Precision Care (or similar databases).
Physical Demands and Work Environment:
The physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position. Physical Demands:
1. Physical lifting, pushing, pulling, stooping, bending associated with working with adults with physical disabilities.
2. Must be able to perform SCIP-R physical techniques and CPR / First Aid skills.
3. Working outdoors during temperate weather and driving in inclement weather as needed.
4. Regularly works with normal household cleaning agents to maintain work site.
5. While performing the duties associated with this position, an employee will be called upon to operate office equipment including but not limited to: laptop and desktop PCs; printers; copiers; projectors; projection screens; sound systems; dvd and and, remote controls for electronic equipment.
6. Employees will regularly be required to stand; walk; sit; use hands (or hands and arms) to carry, finger, handle or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop; crouch and kneel. Employees will also be regularly required to hear (with or without hearing devices), see (with or without corrective lenses) and speak.
Working Conditions and Environment:
While performing the duties associated with this position, an employee will be in an office setting; in residential and other Adapt of the Hudson Valley?program sites; in a van or other vehicle; or, in the community at large (attending seminars, conducting training sessions).
This position requires an employee to use their personal vehicle on a regular basis to travel to Adapt HV sites and elsewhere in performing their job duties. Employees in this position must maintain liability insurance on their personal vehicle that meets the minimum standards specified by the agency. Mileage reimbursement for use of the personal vehicle will be available pursuant to Adapt of the Hudson Valley's?policies.
