Schedule: Part-time | 16 hours per pay period | Every other Saturday & Sunday, occasional holidays| 8 am – 4:30 pm

As a Triage Nurse in Hospice you are responsible for answering incoming calls, triaging patient needs, and coordinating timely care during scheduled hours. This role organizes and directs skilled nursing services in alignment with state and federal regulations, agency policies, and professional nursing standards. By reviewing referrals, determining urgency, and facilitating seamless communication across interdisciplinary teams, this position ensures patients and families receive high-quality, responsive care.

As a Triage Nurse, you will:
- Call Management & Clinical Triage – Respond to all incoming calls during scheduled hours, assess urgency using triage protocols, and determine the best course of action for patient care.
- Referral Review & Admission Coordination – Evaluate new referrals across platforms, determine acceptance based on agency criteria, and schedule admission visits or weekend admissions when appropriate.
- Care Coordination & Communication – Collaborate with patients, families, referral sources, physicians, and interdisciplinary teams, providing updates and ensuring continuity of care.
- Clinical Support & Orders Management – Utilize hospice standing orders to make appropriate medication adjustments, assign PRN visits, and support on-call teams in delivering care.
- Documentation & Compliance – Record all triage activities and interventions in the EMR, maintaining accuracy, regulatory compliance, and alignment with quality standards.

This job also requires:
- Current Registered Nurse (RN) license in Minnesota and North Dakota.
- Hospice experience preferred.
- Strong verbal and written communication skills.
- Ability to pass state mandated background checks.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with team members, clients, and families.
