Description

Hospice of the Piedmont is looking for a Resource RN to manage calls, assess client needs, and coordinate home visits. The position collaborates with Interdisciplinary Group (IDG) members and facilitates communication among the IDG, the patient, and the patient’s Attending Physician.

This position is an office-based/field hybrid position based out of our Charlottesville office. Candidates looking for remote work will not be considered unless they are planning on relocating to the area. 


Responsibilities


1. Phone Triage

  • Perform a telephone or video assessment of patient's needs and/or concerns.
  • Assess the patient and family’s need for a home visit by a clinician or volunteer.
  • Provide guidance, support, and education to Hospice patients and families over the phone. 
  • Document all communication and/or interventions in the patient’s electronic medical record.
  • Communicate with the IDG, the patient’s attending physician and/or the hospice medical director or designee for medical authorizations, pain and symptom management, changes to the Plan of Care, and other exchanges of information. 

2. Support for Clinical Supervisors and Case Managers

  • Pain and symptom follow-up calls
  • Medication refills
  • Supply / DME orders
  • Maintain compliance and quality audits
  • Notification of patient deaths to attending providers  

Qualifications

  • Graduate of an accredited nursing program
  • Current Virginia Registered Nurse license
  • Minimum of three (3) years of experience as a registered nurse within last 5 years
  • Proof of acceptable automobile insurance and reliable transportation
  • Reliable cell phone/Home phone with local number and coverage

Skills & Abilities

  • Expertise or high knowledge in pain management, symptom control, crisis intervention, and family support
  • Demonstrated critical thinking skills and excellent clinical judgment
  • Experience with electronic medical records (EMR)
  • Knowledge of regulatory requirements in hospice
  • Strong organization skills with sharp attention to detail
  • Excellent communication, customer service, and interpersonal skills

Typical Physical Demands/Working Conditions

  • Drive long distances to homes and facilities
  • The job requires regular use of hands for handling objects, tools, or controls, and involves sitting, standing, bending, reaching, lifting, walking, and climbing steps. Vision requirements include close, distance, color, peripheral, depth vision, and the ability to adjust focus 
  • Communication by phone, writing, speaking; Regular use of computer screen and keyboarding are required
  • Lifting to 50 pounds, push up to 200 pounds in a wheelchair.
  • General fast-paced work environment
  • Physical and emotional health is necessary for the accomplishment of duties
