WHO is Sound Generations?

We are a comprehensive non-profit organization that serves older adults and people with disabilities in King County. Our mission is to partner with older adults to provide accessible and inclusive services so they can age their way.

We are currently seeking a Pathways Case Manager to join our assistance services team in Seattle, WA. 

What will YOU do?

The Pathways Case Manager performs independent comprehensive case management that includes screening and assessing clients; creating, implementing and monitoring referrals/care plans; connecting clients to a variety of community resources; and providing client-centered care to help individuals navigate challenging situations. The position is mostly remote but includes occasional required in-person client work.

Pay Rate: $31.82 per hour.

Work Schedule: Monday Through Friday, 40 hours per week.

Responsibilities:

  • Engage clients in comprehensive assessment to identify needs, considering cognitive, behavioral, social and functional aspects.
  • Develop and implement strengths-based care plans to include consulting with client family members to identify appropriate support interventions. 
  • Provide general information/assistance and consultation to clients needing support navigating caregiver support and/or local, state or federal programs addressing the needs of older adults.
  • Facilitate appropriate referrals to community resources and advocate for needed services, providing necessary follow-up to ensure service quality and timeliness.
  • Prepare case file data, documentation and/or reports including case notes supporting eligibility and referral for services and mandated reporting.
  • Review and examine cases on a periodic basis to assess client progress and determine continuing eligibility of programs. 
  • Coordinate delivery of supplemental services/goods in accordance with policy guidelines. To include research, recommendation, delivery, documentation and follow-up. 
  • Provide crisis intervention as a Mandatory Reporter as defined by state law. Immediately report identified concerns such as abuse, neglect, emergent medical issues, etc. to APS or other appropriate authorities.

Requirements: 

  • Bachelor's degree or equivalent combination of education and experience AND three years' experience providing direct human services.
