Position Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to provide in-home care for clients in our private duty home health services. In this role, you will assess patient health needs, evaluate caregiver skills, perform supervisory visits, and collaborate with families and caregivers to promote safety, comfort, and overall well-being. The ideal candidate has strong clinical judgment, excellent communication skills, and a passion for delivering high-quality, personalized care in the home setting.
Key Responsibilities:
- Assess patients’ health status and needs in the home setting.
- Develop and implement individualized care plans in collaboration with patients, families, and caregivers.
- Educate patients and families on disease management, health maintenance, and safety.
- Communicate patient progress, concerns, and changes in condition to supervisors and other care team members.
- Maintain accurate and timely documentation of care in compliance with company policies and regulations.
- Promote a safe, comfortable, and supportive environment for patients in their homes.
- Conduct caregiver evaluations and assess caregivers’ skills to ensure quality of care.
- Perform supervisory visits with caregivers to observe care delivery, provide guidance, and ensure compliance with care plans.
Requirements
Qualifications:
- Active RN license in the state of practice.
- Previous experience in home health, private duty, or outpatient nursing preferred.
- Strong clinical assessment and patient care skills.
- Excellent communication and interpersonal skills to work effectively with patients, families, and caregivers.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and a valid driver’s license.
- Proficiency with electronic medical records (EMR) and basic computer skills.
- CPR and other relevant certifications as required.
- Willingness to travel up to 75% of the time.
