Position Duties
Under general supervision, assess client health problems and needs, develop and implement nursing care plans, maintain medical records, administer nursing care to ill, injured, convalescent, and/or disabled clients. The Care Coordinator may also advise clients on health maintenance, disease prevention, and provide case management.
- Schedule enrollment visits, annual visits, and 6-month updates as required by programmatic policies & requested by families & clients.
- Mandatory travel to complete home visits within the North Central Health District’s 13 counties. Home visits are completed at least twice annually per client (the frequency of visits is subject to change dependent upon the needs of the client and/or family).
- Document all encounters with families and clients in designated client database, including all communications with internal and external vendors, providers, and community partners.
- Provide care coordination as stated by programmatic policies including however, not limited to being a liaison between families and providers/vendors and transitioning families from pediatric to adult care according to CMS policy.
- Comply with all HIPAA policies provided by the Department of Public Health via the NCHD Intranet.
- Complete and document prior authorizations as requested by families and approved by CMS policy.
- Scan, review, and enter patient information provided by medical providers.
- Contact providers to assist with understanding of medical diagnoses, obtaining Rx instructions, prior authorizations, and any other pertinent information necessary to maintain the patient’s continuity of care.
- Research & provide resources as needed and/or requested by families.
- Update and maintain telemedicine clinic equipment & supplies for clinic use.
- Attend meetings, health fairs, conferences, workshops, and other outreach events as needed and/or requested.
- Maintain nursing license requirements including acquiring CEUs.
- Provide admin and/or clinical services in authorized shelters as requested by Emergency Preparedness during a public health state of emergency.
- Assist in telemedicine clinics monthly including handling telemedicine cart & equipment, clinical documentation, phlebotomy, and other lab collection processes.
Minimum Qualifications
Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lowerlevel PH Registered Nurse 1 (HCP190) or position equivalent.
Preferred Qualifications
A nurse licensed in Georgia with a minimum of one year of experience working in the delivery of services to children with special health care needs and/or women’s health; and a professional (Bachelor or Master Level) with one year of human service delivery experience in health and human services. A licensed nurse who also has home visiting experience.
