Develop strategies to link residents with health care and care coordination services. Develop relationships with the primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization. This is a grant-based position. This position is not eligible for a hybrid work schedule.
The Margot & Harold Schiff Residences are located in Chicago’s Near North neighborhood. The property includes 96 single-room units available to formerly homeless people, many of whom have special needs and/or a history of substance abuse. This property breaks new ground in the application of green design strategies and the use of energy-efficient building systems.

Duties:
- Maintain detailed knowledge of the health care system, key service providers and health care delivery system developments related to the health needs of residents.
- Collaborate with Resident Service Coordinators (RSC) to identify residents most in need of care-coordination and self-management assistance and determine what staff member is most appropriate to provide this assistance.
- Accurately identify/assess resident assets and needs to connect them effectively to resources and help them set personal goals, while recognizing their knowledge and experience.
- Develop partnerships with the primary care clinics, medical providers, hospital discharge planning team, care coordinators, health plans and other health service providers who work with the largest numbers of residents at high risk of health problems and elevated health care utilization (“high health risk”).
- Implement procedures for consented exchange of information to facilitate collaborative care coordination and effective health self-management by residents.
- Work one-to-one with selected high-risk residents to increase their engagement with health care, care coordination and wellness services.
- Assists with grant management and compliance reporting
MINIMUM QUALIFICATIONS
- High School Diploma or equivalent.
- Two (2) years of experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.
PREFERRED QUALIFICATIONS OF POSITION
- Bachelor’s Degree in Healthcare Management, Public Health, or related field.
