About the Role: 

Community Health Worker (or Promotor in ¡Vamos! Health) will play a crucial role in promoting health and wellness to the patients we serve. This position involves working closely with community members to provide culturally competent health education, support, and resources. The CHW or Promotor will act as a liaison between the healthcare provider and the community, facilitating access to services and addressing the social determinants of health. 

 

This role will report to the Community Health Lead. 

 

Essential Duties and Responsibilities: 

  • New Patient Support 
  • Patient onboarding and account creation 
  • Appointment coordination. 
  • Help patients to understand medical instructions and
    accessing resources.
     
  • Act as a liaison between patients and providers to ensure effective communication and understanding.  
  • Conduct SDOH and Needs Assessment surveys. 
  • Provide emotional support and encouragement to patients and their families. 
  • Address client issues and barriers to care promptly, finding solutions to improve access and quality of services. 
  • Adhere to all clinic policies, procedures, and ethical guidelines, ensuring client privacy and confidentiality. 

 

  • Building Relationships 
  • Establish
    and maintain trusting, respectful
    relationships with patients and key stakeholders within community.
     
  • Adapt communication to resonate with community. 
  • Foster a respectful and inclusive environment that values diversity. 
  • Key point of contact for patient needs 
  • Provide emotional support and encouragement to clients, fostering a sense of trust and safety. 
  • Educate clients about available health services and resources within the community. 
  • Healthcare Navigation 
  • Understand the overall healthcare system, community resources, resources provided by ¡Vamos! Health, and how they work together.  
  • Provide resources and education for patients. 
  • Utilize platforms to distribute educational materials, videos, and resources to clients. 
  • Update patient resource plans based on changing needs and circumstances. 
  • Organize and conduct virtual/in person health workshops, webinars, and support groups. 
  • Address client issues and barriers to care promptly, find potential solutions to improve access and quality of services. 
  • Data Collection & Reporting 
  • Accurately enter and update client information, medical histories, and visit notes in the implemented systems.   
  • Document patient interactions accurately and confidentially (HIPAA guidelines) 
  • Compile and report data on patient health trends, needs, and outcomes. 
  • Participate in regular team meetings. 
  • Regularly provide insights and feedback to Patient Care Lead about patient experiences or concerns 
  • Ongoing Patient Engagement  
  • Engage in care planning and care plan management.  
  • Follow up on referrals to specialists and procedures with outside organizations by confirming appointment dates, ensuring patients have transportation, reminding them of their appointments, and documenting processes as required. 
  • Resolve barriers to accessing resources.  
  • Manage communication with patients on ongoing membership, medical, and community-based needs. 
  • Complete assigned certifications, trainings, and education to enhance patient engagement and health condition
    management. 
     
  • Other Duties as assigned 

 

Qualifications 

  • B1/C2-proficiency in English and Spanish (written and spoken) 
  • At least 3 years of experience working in a community outreach or health education role. 
  • Knowledge of health promotion, disease prevention, and community resources 
  • Strong interpersonal and communication skills 
  • Ability to work independently and as part of a team.

    Work Conditions: 

    • Generally, in an indoor setting 
    • Frequent use of a computer 
    • Varying schedule to include evenings, holidays, and extended hours as business dictates. 
    • While performing the duties of this job, the employee is occasionally exposed to heat or cold temperatures.
