ORGANIZATION DESCRIPTION

Founded in 1965, the Chinese-American Planning Council, Inc. (CPC) aims to promote the social and economic empowerment of Chinese American, immigrant, and low-income communities. Today, CPC is the nation's largest Asian American social services agency and offers more than 50 programs at over 35 sites throughout New York City, including early childhood education, school-age care, youth services, workforce development, community services, and senior services. CPC strives to be the premier social service and leadership development organization. In 2017, CPC launched Advancing Our CommUNITY, its organization-wide strategy to expand services to address persistent needs and emerging trends and to improve leadership skills among staff and community members. In 2022, our core programs and emergency services reached 200,000 people living in all five boroughs and all 51 city council districts. PROGRAM SUMMARY

CPC Senior Services division is currently seeking a full-time Case Worker for its Older Adult Center. This program offers congregate meals, evidenced based health and wellness program, English as a Second Language (ESOL), painting and other arts, music and cultural classes, translation support and advocacy, referrals and SNAP, health insurance, and housing assistance, and other case management supports. Our programs have been active in remote programs and helping ensure ongoing wellness calls and connection to meals/nutrition aid/groceries delivery were available for seniors throughout the pandemic. Celebrations, and other social and cultural events and activities keep seniors active in mind, body, and spirit.

 

 

JOB SUMMARY

 

Reporting to the Program Director, the full-time Case Worker will be providing older adult services including, information referral, benefits support, case assistance, and providing support to the Program Director to ensure high quality services that respects the dignity of all the community members.

 

 

ESSENTIAL RESPONSIBILITIES

 

  • Assisting older adults with applications to entitlement programs.
  • Assessing cases and compiling case reports containing relevant information.
  • Ensuring that case reports are kept up to date.
  • Providing support, guidance, and counselling to older adults in need.
  • Making referrals or introductions to other agencies.
  • Scheduling appointments with legal aid workers, counselors, doctors, etc.
  • Monitoring visits for at-risk or fragile members.
  • Motivating individuals to participate in rehabilitation programs.
  • Reporting abuse.
  • Maintain participant files/programmatic documents; track participant attendance in services and progress notes; compile program reports as needed by the Director.
  • Report to and fulfill other related duties as outlined by the Director


QUALIFICATIONS

Education and Experience:

  • Proven coordination skills to connect individuals/families with relevant services.
  • Ability to compromise, negotiate, and work well with others.
  • Training in Education, Psychology, Sociology, Social Work, or related field preferred.
  • Prior experience working with older adults in a nonprofit or related setting.

 

Skills and Competencies:

  • Strong work ethic and demonstrated passion for senior services.
  • Excellent communication, organizational and interpersonal skills; knowledge of older adult and immigrant issues helpful.
  • Must be socially perceptive to work with older adults.
  • Knowledge and skills using Outlook, Word and Excel.
  • Fluency in Chinese (Mandarin and/or Cantonese) required.

 

 

COMPENSATION & BENEFITS OVERVIEW

 

  • $21.00 - $25.00; 35 HOURS PER WEEK
