Location: Houston, TX | Full-Time | $23-$25/hr

Summary
The Field Memory Connect Wellness Coordinator (MCWC) plays a vital role in bringing supportive services to residents across multiple partner senior living communities while strengthening engagement with the Senior Doc Primary Care and GUIDE Dementia Support Programs.
As a field-based professional, the MCWC travels daily to assigned communities, fostering meaningful relationships with residents, families, and staff. This role serves as a key point of contact for patients with memory-related conditions, ensuring care is well-coordinated, communication is seamless, and quality of life is enhanced through compassionate, personalized support.
Responsibilities
Travel daily to assigned senior living communities to provide on-site support.
Support dementia care activities through the GUIDE program.
Provide in-person assistance during telehealth provider visits.
Assist with care coordination, care plan updates, and patient enrollment.
Facilitate communication between patients, caregivers, providers, and community staff.
Provide companionship and engagement opportunities to residents.
Participate in community and company events to promote wellness and connection.
Support event, marketing, and administrative needs across community initiatives.
Perform additional field-based duties as assigned.
Requirements
At least 18 years of age.
High school diploma, GED, or equivalent (required).
Medical Assistant Diploma (preferred).
- Bilingual (preferred)
Education or experience in patient care or case management.
Outgoing, professional, and self-motivated qualities.
Ability to work independently in the field.
Ability to stand/walk for extended periods of time.
Reliable transportation with willingness to travel daily to assigned locations.
Flexibility to attend/work community events outside a typical M–F, 8–5 schedule.
Phlebotomy License (preferred).
