Field Memory Connect Wellness Coordinator (MCWC)

Location: Houston, TX | Full-Time | $23-$25/hr

Summary

The Field Memory Connect Wellness Coordinator (MCWC) plays a vital role in bringing supportive services to residents across multiple partner senior living communities while strengthening engagement with the Senior Doc Primary Care and GUIDE Dementia Support Programs.

As a field-based professional, the MCWC travels daily to assigned communities, fostering meaningful relationships with residents, families, and staff. This role serves as a key point of contact for patients with memory-related conditions, ensuring care is well-coordinated, communication is seamless, and quality of life is enhanced through compassionate, personalized support.


Responsibilities

  • Travel daily to assigned senior living communities to provide on-site support.

  • Support dementia care activities through the GUIDE program.

  • Provide in-person assistance during telehealth provider visits.

  • Assist with care coordination, care plan updates, and patient enrollment.

  • Facilitate communication between patients, caregivers, providers, and community staff.

  • Provide companionship and engagement opportunities to residents.

  • Participate in community and company events to promote wellness and connection.

  • Support event, marketing, and administrative needs across community initiatives.

  • Perform additional field-based duties as assigned.


Requirements

  • At least 18 years of age.

  • High school diploma, GED, or equivalent (required).

  • Medical Assistant Diploma (preferred).

  • Bilingual (preferred)
  • Education or experience in patient care or case management.

  • Outgoing, professional, and self-motivated qualities.

  • Ability to work independently in the field.

  • Ability to stand/walk for extended periods of time.

  • Reliable transportation with willingness to travel daily to assigned locations.

  • Flexibility to attend/work community events outside a typical M–F, 8–5 schedule.

  • Phlebotomy License (preferred).
