- Administers long acting injectable medication and supports clients with medication adherence.
- Coordinates laboratory services and reports.
- Documents all services and progress notes in electronic health record.
- Implements treatment plans, revising and updating as necessary, in collaboration with the ACT team.
- Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the client.
- Transports and utilizes required medical equipment and supplies weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, TGCW offices and other locations.
- Supports program RN in managing all aspects of the psycho-pharmacological treatment of ACT clients, in collaboration with, and under the direction of, the ACT MD.
- Carries a caseload, and provides general case management services in addition to any specialized services needed by clients.
- Provides nursing care in accordance with Agency policies, practices, procedures, Standards of Nursing Practice, and Assertive Community Treatment Program guidelines.
- Provides a range of treatment, rehabilitation, and support services.
- Provides any of the following services to clients: advocacy, wellness self-management, harm reduction treatment, monitoring of mental status, emotional support, counseling, family based interventions and crisis intervention.
- Makes appropriate community referrals for services as required such as referring clients to community physicians and maintaining contact with primary care providers.
- Supports RN with developing and leading health education classes on topics such as diabetes, heart disease, substance use, asthma, nutrition, wellness, medication self-management, etc.
- Assists clients when necessary by escorting them or arranging for escort to appointments and interviews.
- Coordinates with pharmacies to ensure timely delivery of medication to the ACT office or the client’s home.
- Takes on call shifts, available to respond to calls to the ACT emergency phone 24/7 during week-long shift.
- Maintains productivity sufficient to meet ACT Team standards.
- Maintains records in accordance with OMH and agency record keeping standards.
- Supports RN with ordering and storing medical supplies.
- Participates in interdisciplinary team meetings, case conferences, clinical supervision, and staff development trainings
- LPN license is required
- HS Diploma or Associates degree is required
- 1 year of experience of providing direct services related to treatment and recovery of individuals with serious mental illness
- Knowledge of electronic health record
