Responsibilities include, but are not limited to:
- assessing/reassessing, developing, coordinating, and monitoring specialized services for individuals living in nursing facility diagnosed with Intellectual/Developmental Disabilities (IDD);
- Meeting face to face with each individually monthly
- Facilitating in-person meetings for each individual at least quarterly;
- Being highly organized to meet strict timelines for documentation;
- Identifying alternate placement options when applicable;
- Negotiating and advocating for individuals;
- Developing effective working relationships with the individuals, family members, nursing facility staff, and other service providers;
- Effective verbal and written communication
- Being part of a team;
- Traveling to Ellis, Kaufman counties (and occasionally within the 12 county service area)
- Conducting PASRR evaluations as needed
- Working from designated office, home, and in the field (nursing facilities in designated area)
Salary: $19.00
Work Schedule: 8-5 (Mon-Fri)

Education:
Bachelor's or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field, such as psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.
Work Experience:
- 1 year of experience working with individuals who have intellectual or developmental disabilities.
- Knowledge of psychotropic medications and DSMV
- Proficient computer skills, including MS Word, Excel and Outlook
- Bilingual skills a plus (English, Spanish)
