Newly created position due to LHCSA services growth in CNY and Catskill region. Will require paid travel throughout the CNY region as well as Otsego, Delaware and Schoharie Counties.
Role and Responsibilities
The RN Care Services Supervisor is responsible for managing client care as efficiently and cost-effectively as possible, to achieve positive patient outcomes regardless of payer. This role is also highly focused on Aide retention operates within the Licensed Home Care Service Agency (LHCSA). The RN Care Services Supervisor is a dedicated to participating in our Home Health Aide Training Program (HHATP) teaching new staff to be Aides. They will follow Aides from the classroom to the field so they can transition seamlessly and apply learning to hands on patient care. This role will be responsible for developing the overall plan of care for clients and the outcome of that care, including care delivered by other staff (LPN, HHA, PCA) who must be directed and supervised by the LHCSA RN.
Essential Functions
RN Case Management Functions:
Overall responsibilities include: providing and coordinating quality patient care while maintaining a case load/productivity as assigned by direct supervisor. Accurate and timely documentation supporting care provided and/or needs of the client.
- Conduct admission assessments and determine if a safe plan of care can be administered including the need for home health aide/personal care assistance including time of day, frequency of visits and length of time required to complete the plan of care.
- Case manage primarily aide-related services according to regulatory guidelines.
- Conduct in-home supervisory visits to oversee care given by home health aides and personal care aides.
- Practice according to best practice standards and regulatory nursing standards.
- Maintain relationships and clear lines of communication with MLTC case managers and other members of the care team.
- Communicate effectively with aides, provide mentoring & positive feedback through phone conversations, and visits
- Oversee clinical skills of assigned aides, provide training.
- Communicate with aide preceptor regarding skills, possible support needed in field.
- Document according to standards of nursing practice.
- Contact the primary physician and/or notify the clinical supervisor per regulatory guidelines.
Aide Supervision and Education Functions:
- Mentor new hires with consistent oversight
- Provide leadership, supervision, and operational guidance to Home Health Aides and Personal Care Aides, including reviewing Aide productivity, performance management, employee relations, and disciplinary/counseling.
- Assist in completing annual performance evaluations
- Investigate and address Aide concerns, complaints, accidents, and other issues involving peers, patients, and/or care delivery. Report to the Regional Director, Executive Director and/or Human Resources, as appropriate.
- Assist clinical educator in executing actions for Aide’s development of knowledge and skills, to ensure proficiency and patient care quality.
- Responsible for ensuring all assigned staff complete all in-service credits annually.
- Assist in coordination of HCR Preceptor program
- Demonstrate competencies as they relate to information/technology.
- Establish and maintain relationships with community partners.
- Other duties as assigned.

Education Requirements
- Bachelor’s Degree of Nursing preferred.
Qualifications and Requirements
- Current, valid RN license issued by NYS Department of Education required.
- Minimum of two (2) years’ experience in home care.
Work Environment
This role is primarily in a non-office setting and may be exposed to outdoor conditions.
The working conditions are classified as heavy work:
- Heavy work - Exerting up to 100 pounds or more of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Heavy work involves sitting and standing.
