The Licensed Practical Nurse (LPN) The Licensed Practical Nurse performs a wide variety of duties and responsibilities in a manner that places emphasis on quality of care and customer service. They provide services directly and collaborate with providers as well as outside agencies to ensure a patient centered approach to care. As part of the essential functions of this role, the Licensed Practical Nurse:
- Collaborates with and supports team members to ensure a patient-focused approach to care
- Provides outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
- Assesses, plans, implements and evaluates care for patients under their scope of practice. Prepares patients for provider visits by taking vital signs, history and assessing risk factors
- Collects and records health information findings of physical and developmental assessments into patient record with timeliness and accuracy; accesses and retrieves medical records via HEALTHeLINK™
- Responsible for screening all new and returning patients by completing an intake process; comprehensively documents intake encounters and health history into our EMR system
- Facilitates lab and diagnostic tests such as injections, urine and oral toxicology’s at the request of the provider
Qualified Candidate will have current LPN license and current BCLS certification with a minimum of one (1) year of experience working in a medical practice setting, preferably in an HIV/AIDS or other infectious disease practice. Prior phlebotomy experience required. Candidate must be able to communicate clearly and professionally in writing and verbally. Candidate must demonstrate familiarity with computers and MS Office software (Outlook, Word) and electronic charting/medical records systems (Medent preferred). Candidate must demonstrate personal responsibility and integrity and ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential.
