The Regional Geriatric Program Central (RGPc) in partnership with Hamilton Health Sciences; St. Joseph’s Healthcare Hamilton; Niagara Health System; Alzheimer Society Brant, Haldimand Norfolk, Hamilton, Halton; Community Addictions and Mental Health Haldimand and Norfolk; is leading the implementation of the Specialized Geriatric Services (Geriatric Medicine; Geriatric Psychiatry, Mental Health) and Behavioural Supports Ontario Coordinated Clinical Intake. The coordinated clinical intake service will provide a central intake for referrals, with a focus on benefiting older adults, families, primary care providers and the specialized service recipients of the referrals.
The Clinical Intake Coordinator will work collaboratively with the HNHB Community based Specialized Geriatric Services which includes, Geriatric Medicine and Geriatric Psychiatry/Mental Health in addition to the Community Behavioural Supports Ontario (BSO) Teams.
The Clinical Intake Coordinator will:
- Assess appropriateness of referral for service, including the redirection of referrals to appropriate alternate services;
- Coordinate case conferences with relevant community services and resources in determination of most appropriate referral and assignment to SGS;
- Work in collaboration with the team and service providers to ensure seamless access to services.

Monday – Friday Days (may require evenings or weekend on occasion)
*!• Undergraduate degree in the field of health care is required;
• Member in good standing of a regulatory college;
• Additional experience, training, and/or education in the field of geriatrics/geriatric psychiatry is required;
• A minimum of three years of clinical experience working with older adults required;
• Strong clinical assessment and reasoning skills is required;
• Critical thinking skills and a creative problem-solving approach is required;
• Demonstrated knowledge of the HNHB LHIN system of care and services for seniors and ability to navigate health and social care systems is required;
• Exhibits an ability to provide an exceptional person centred service approach with the public;
• Proven advanced organizational skills and an ability to work with minimal supervision;
• Demonstrated ability to work in a highly collaborative team approach;
• An ability to advocate for the unique needs of individual seniors within a patient centered philosophy;
• Strong communication and conflict management skills;
• A working knowledge of PIECES framework is an asset;
• Working knowledge of computers (Microsoft Office, Meditech, Clinical Connect, CHRIS, etc.) data entry, computer trouble-shooting;
• Knowledge of the following would be an asset: Health Care Consent Act, Substitute Decision makers Act, Mental Health Act, Personal Health Information and Privacy Act.
