Awarded "Best Senior Living, 2024 & 2025" by U.S. News & World Report, Las Ventanas at Summerlin, a HumanGood community highly regarded for its quality of care in Assisted Living, Independent Living, and Memory Care, is seeking an experienced and skilled Wellness Nurse (LPN) for its Independent Living residents. Our Licensed Vocational Nurses/Licensed Practical Nurses (LVN/LPN) are responsible for the nursing needs of residents, while actively incorporating our “Philosophy for Person-Directed Care.” The Residential Living Nurse must exhibit genuine interest in residents and show patience and understanding while working with residents, families and visitors. This position assesses health, functional and psychosocial status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in the independent living environment. The Residential Living Nurse oversees and promotes the residents' health and wellness through regular visits with each resident, resident assessments, and ongoing communication with the resident care team.

This position is FULL TIME, Days - 1 weekend day availability is a MUST. Salary is $28.75 to $37.25 DOE.

Key duties and responsibilities include, but not limited to:

  • Is the first responder for residents in independent living in the event of a medical need.
  • Assesses resident’s needs at the time of move in and continually evaluates residents’ condition. Promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community’s safety procedures);
  • Facilitates nursing care provided to residents and includes residents in decision making processes;
  • Utilizes the nursing process to identify resident conditions that require further evaluation and treatment by a physician;
  • Reports significant medical changes to the relevant Team Members, Physician, Director of Wellness and designated family members as appropriate;
  • Responds to emergency situations, carries out physician orders, and documents the event, interventions, and outcomes;
  • Accurately and completely performs direct nursing services, such as, medication administration, treatments, personal care and other nursing procedures.
  • Accurately receives documents and transcribes Physician Orders with the appropriate follow-through in a timely fashion, specifically advocating resident safety;
  • Assures proper documentation including but not limited to: consents, summary notes, appraisals, service plans, medication records, incidents, etc.
  • Reports and documents vital information on all guests’ paperwork;
  • Coordinates the residents’ plan of care with the interdisciplinary team (IDT), physicians, medical personnel, and resident’s family/responsible party;
  • Makes necessary documentation corrections as a result of electronic health record and medical record audits;
  • Builds genuine relationships with residents through discussion and review of their history;
  • Checks on operating condition of equipment and notifies appropriate personnel when necessary;
  • Provides effective and courteous service to all residents, family members, guests, and team members; is clear in communication and handles conflicts appropriately;
  • Assists in Resident and family education;
  • Updates the Director of Wellness on any administrative concerns, such as family issues and the like.
  • Ensures that the community Executive Director and Associate Executive Director are notified of emergency situations that occur after hours or on weekends.


To be successful in the role, you would have:

  • Current LVN/LPN (or obtaining within 90 days) in the state for which applying
  • Current CPR certification
  • Prior nursing experience: senior care, skilled nursing, post-acute or sub-acute care preferred
  • Clinical observation and assessment skills
