Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.

As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.

The Rehabilitation Supervisor will oversee the daily operations of the rehabilitation therapy functions. This role will provide and monitor the team's case management, treatment, and service coordination with participants to ensure compliance with state and federal regulations. Additionally, this role will interact, coordinate, and facilitate activities with all disciplines, departments, and appropriate regulatory agencies.

Schedule: Monday-Friday; 8:00 am-4:30 pm. This position is onsite. 

Responsibilities

  • Leads, coordinates, and supervises licensed therapy professionals and technicians, ensuring continuity and consistency of participant care planning
  • Orients new staff to the PACE model, CMS, regulatory requirements, and ensures compliance with PACE guidelines; provides ongoing coaching and training
  • Assists with policy and procedure solutions to develop department best practices
  • Develops and implements occupational therapy programs for patients accessing day services
  • Conducts patient assessments to determine physical abilities and mobility
  • Develops and implements individualized plans of care to decrease/eliminate disability, increase/maintain capability for independence, and develop function to a maximum level
  • Guides patients and caregivers in the use of adaptive devices, therapeutics, and creative self-care activities for improving function
  • Evaluates the effectiveness of treatment plans and modifies goals, as needed
  • Conducts home, hospital, and other site visits as indicated by IDT
  • Records and signs treatment plans in patient health records
  • Records, signs, and dates quarterly progress notes, including reviews and initials on evaluations and discharge summaries
  • Ensures services provided are in a therapeutic setting and conducive to restoring dignity and self-esteem to patients
  • Manages required equipment and supplies for the occupational therapy program to ensure availability to patients; order equipment, as needed
  • Participates in training programs for staff and caregivers
  • Identifies and collects appropriate data as a means of measuring interventions, outcomes, and regulatory compliance
  • Ensures delivery of quality services by overseeing the department's adherence to policies/procedures and compliance with state and federal regulatory requirements
  • Attends and participates in staff meetings, organizational QI activities, and assigned committees, including participant care planning, utilization reviews, ethics, grievances, and appeals
  • Organizes and facilitates department meetings, IDT, and huddles
  • Completes all required training programs and participates in ongoing conference calls, webinars, and other professional development opportunities
  • Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies, such as PACE, CMS, and DHCS

Leadership/Management & Supervision

  • Works with management to develop and implement policies, protocols, and procedures, including staff training to ensure the team is delivering accurate, appropriate, and consistent service and information to participants
  • Ensures the highest levels of business practices are in place and continuously monitor workflows and processes
  • Recommends or make decisions to hire, transfer, reward, discipline or terminate employees
  • Provides coaching, training, and staff development feedback to employees
  • Manages workforce planning for the Therapy Department, including new role development
  • Nurtures employee growth and development to enable employees to focus their efforts on direct patient care management
  • Contributes to the success of the organization by participating in quality improvement activities
  • Responds promptly and with caring actions to participants and employees
  • Maintains professional working relationships with all levels of staff, clients, and the public
  • Leads and strives to ensure department goals and objectives are achieved
  • Functions at highest level according to credentials and competency

Qualifications

Education/Experience

  • Education: must possess one of the following:
    • Graduate of an accredited Physical or Occupational Therapy program AND a valid CA Physical or Occupational Therapy License 
    • Graduate of an accredited Physical Therapy Assistant program AND a valid CA Physical Therapy Assistant License 
    • Graduate of an accredited Certified Occupational Therapy Assistant program AND a valid CA Certified Occupational Therapy License 
  • Valid BLS certification from approved American Health Association provider required upon hire
  • One year experience working with the frail elderly population required; experience working in PACE environment preferred
  • Two years of physical therapy or occupational therapy experience required
  • Supervisory experience preferred
  • Completion of LEAP Program preferred (Internals Only)

Additional Qualifications (Knowledge, Skills and Abilities)

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with providing therapy to geriatric participants
  • Ability to apply analytical thinking and problem-solving to resolve issues
  • Ability to travel between site in multiple regions
  • Excellent project management, planning, and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

  • Ability to lift/carry 15 lbs/weight
  • Ability to stand for long periods of time

Salary Ranges

  • PT/OT: $104,200-$156,200 annually, depending on experience
  • PTA/COTA: $80,200-$115,500 annually, depending on experience

While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
