PRI, Registered Nurse
Department: ** Location: Syracuse, NYThe registered nurse performs home care evaluations that require standardized assessment tools such as but not limited to PRI�s, Screens assessments for the purpose of establishing eligibility for specific programs that require such documentation.
Performance Responsibilities and Standards:
1. Ensure quality and safe delivery of home health services.
- Registered Nurse is able provide care to a diverse set of clients.
- Contact and schedule visits with clients in a timely manner.
- Communicate findings to appropriate parties as identified during referral process.
- Determine appropriate scoring based on client assessment.
- Documentation reflects nursing care and interactions.
- Ensure that all documentation meets quality standards.
- Documentation is clear and concise.
- Documentation is completed in timely manner
- Understand and follow nurse code of ethics and standards of practice.
- Coordinate with internal and external resources to meet patient needs.
- Maintain established productivity expectation
- Communicate client care issues requesting facility/care organization.
2. Comply with Nascentia Health policies and procedures and quality initiatives.
- Maintain confidentiality.
- Demonstrate knowledge and use of infection control practices.
- Demonstrate knowledge of federal, state and local government rules and regulations.
- Demonstrate knowledge and compliance with agency policies and procedures.
- Adhere to quality improvement standards.
- Keep personnel requirements up-to-date; i.e., health requirements, licensure requirements, and other Nascentia Health requirements.
- Demonstrate skills competency through periodic evaluations.
3. Participate in personal growth and development.
- Participate in educational in-services and meetings.
- Maintain and update clinical knowledge on an ongoing basis.
Qualifications:
- License: Registered Nurse (RN) licensed and in good standing in New York State
- Education: Associate degree in Nursing required; BSN preferred
- Experience:
- Minimum 1 year of nursing experience preferred
- Experience with UAS-NY assessments strongly preferred
- Knowledge of long-term care, home care, Medicaid/Medicare preferred
- Skills:
- Strong verbal and written communication skills
- Proficiency in Microsoft Office and ability to learn EMR systems
- Independent, detail-oriented, and able to manage a flexible schedule
- Bilingual abilities a plus
- Other:
- Valid NYS driver�s license and reliable transportation
- Ability to drive and work in various environmental and weather conditions
Work Environment & Physical Requirements:
- Hybrid role: Primarily home-based with required in-home member visits and occasional in-office meetings
- Must be able to sit, walk, stand, drive, and perform data entry throughout the day
- Ability to lift up to 20 lbs frequently and up to 50 lbs occasionally
- Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
- Requires sensory acuity for communication, observation, and documentation
