Pay Rate
$24.85/hr

Your Experience Matters

We’re looking for a Case Manager, a compassionate, dependable person who can walk alongside individuals as they work toward stable housing, greater independence, and better health. Whether you're an experienced case manager or someone with powerful lived experience, your knowledge and empathy are valuable here.
What You’ll Do
You’ll build trusting relationships with people who’ve experienced homelessness, mental health challenges, or other barriers to stability. Your role will be to support and advocate for them as they work toward goals like:
- Staying housed long-term
- Improving life skills and income
- Feeling more in control of their lives
Key responsibilities:
- Get to know each person and help them create a plan for what they want in life—starting with stable housing and building from there.
- Help people connect to services and supports, from health care and jobs to food and transportation.
- Walk with people through complex systems (like benefits or housing programs), offering tools, coaching, and encouragement along the way.
- Be creative and persistent in reaching people who may not feel ready for support, you’ll help build that bridge.
- Support people’s independence by offering clear, respectful information so they can make informed choices.
- Be present and proactive, help prevent crises before they happen and respond quickly when they do.
- Keep notes and records so we can track progress and keep improving our support.
- Share your insights and voice in meetings and planning sessions—your perspective matters.
Requirements
This Role Could Be a Great Fit If
- You’ve worked in social services, peer support, or community programs, and know how to support others facing barriers.
- You’re a natural listener, someone who meets people where they’re at without judgment.
- You’re organized and reliable, you follow through and can juggle a lot without losing the human touch.
- You’re comfortable learning about public systems, benefits, and services, and helping others navigate them.
- You have a valid unrestricted driver’s license.
Preferred Qualifications
- A Bachelor’s degree in a human services field
- At least 3 years’ experience supporting people with disabilities or complex needs
- Knowledge of Orange County resources and community programs
- Strong communication skills (written and verbal)
