Summary Job Description
The Family Care Coordinator (FCC) is responsible for the oversight and coordination of services for individuals placed in Family Care homes, ensuring the delivery of person-centered, trauma-informed, and community-integrated services by the New York State Office for People with Developmental Disabilities (OPWDD). Working under the direction of the Director of Programs for People with Intellectual Disabilities, the FCC oversees the team that supports both individuals in care and Family Care providers, serving as a liaison between families, service providers, and regulatory agencies.
This position is full-time and is based primarily in Little Flower’s Wading River office with a hybrid remote schedule. The role may require travel to client homes and other offices to address client issues and supervise program staff as needed to ensure management of services provided in Queens, Suffolk and Nassau counties, as well as other business-related locations when needed. From time to time, this position requires work outside of normally scheduled working hours.

Our staff of more than 500 builds well-being by providing prevention services, foster care, residential treatment care, adoption services, programs and services for individuals with intellectual and developmental disabilities, and medical and mental health services.
Principal Responsibilities
- Monitor and support the health, safety, and well-being of individuals in Family Care homes through supervision of home liaisons, nurses, behavior specialists and other team members, and direct contact as needed per OPWDD requirements.
- Oversee the team’s work to ensure timely development, review, and updating of Life Plans in collaboration with Care Coordination Organizations (CCOs), providers, and family members.
- Coordinate all clinical, habilitative, behavioral, and community-based services to ensure comprehensive care.
- Provide coaching, guidance, and support to Family Care staff and providers on documentation, behavioral interventions, and crisis de-escalation.
- Track, maintain, and audit documentation (e.g., ISP, MARs, habilitation logs, incident reports) for compliance with OPWDD and Medicaid regulations.
- Collaborate with internal departments (Quality Assurance, Finance) to ensure interdisciplinary team alignment.
- Participate in regulatory audits, surveys, and incident reviews.
- Facilitate initial and ongoing training for Family Care providers and ensure recertification compliance.
- Engage in recruitment and matching individuals to appropriate Family Care settings in coordination with the referral and intake team.
- Respond to and report incidents per OPWDD Part 624 requirements.
Required Qualifications and Experience
- Bachelor’s degree in social work, Human Services, Psychology, or related field required; master’s degree preferred.
- At least 5 years of experience working with individuals with developmental disabilities.
- Working knowledge of OPWDD regulations and HCBS Waiver services.
- Familiarity with Life Plans, habilitation services, and CCO coordination.
- Valid NYS Driver’s License and ability to travel across the Region (Brooklyn, Queens and Long Island.)
- Strong organizational, communication, and documentation skills.
- Bilingual (Spanish/English) preferred but not required.

Salary Range
The salary range for this role is $75,000 to $95,000 with a comprehensive benefits package. Pay rate will be commensurate with knowledge and experience.
Travel Requirements & Physical Demands
- Periodic travel to Family Care homes and other Little Flower locations and to regional meetings or trainings.
- Flexible schedule required to meet the needs of individuals and providers, including occasional evenings or weekends for emergencies.
- Documentation and record-keeping completed through electronic systems.
