The Quality Coordinator in the primary care setting is responsible for implementing, monitoring, and coordinating quality improvement initiatives to enhance patient care, safety, and operational efficiency. This role works closely with healthcare providers, administrative staff, and leadership to ensure compliance with regulatory requirements, support data-driven decision-making, and promote continuous quality improvement.

Requirements

Key Responsibilities:

  • Quality Improvement and Monitoring:
  • Develop, implement, and monitor quality improvement projects to ensure high standards of patient care.
  • Collect, analyze, and interpret clinical and operational data to identify trends, gaps, and areas for improvement.
  • Track and report key performance indicators (KPIs), including patient satisfaction, clinical outcomes, and operational efficiency metrics.
  • Conduct root cause analyses (RCAs) and develop action plans to address identified quality or safety concerns
  • Ensure compliance with federal, state, and local regulations, as well as accreditation standards (e.g., NCQA, Joint Commission, etc.).
  • Prepare and assist with audits, inspections, and quality assessments.
  • Maintain documentation and reporting to demonstrate compliance with quality and performance standards.
  • Provide education and training to clinical and administrative staff on quality improvement processes, best practices, and compliance requirements.
  • Facilitate team meetings to review quality performance and collaborate on improvement strategies.
  • Serve as a liaison between the primary care practice and internal/external stakeholders, including patients, providers, and regulatory agencies.
  • Partner with healthcare providers and staff to implement evidence-based practices and protocols to improve care delivery.
  • Communicate quality initiatives and progress to leadership, ensuring alignment with organizational goals.
  • Ensuring that quality improvement efforts are focused on improving the patient experience and delivering patient-centered care.
  • Identifying opportunities to reduce healthcare costs while maintaining or improving quality within the value-based care framework.
  • Keeping abreast of the latest trends, best practices, and regulatory changes related to quality improvement and value-based care.

Qualifications:

Education and Experience:

  • Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or a related field (Master’s degree preferred).
  • 2-5 years of experience in quality improvement, healthcare operations, or a related role in a primary care or outpatient setting.
  • Knowledge of quality improvement methodologies (e.g., Lean, Six Sigma, Plan-Do-Study-Act).

Skills and Competencies:

  • Strong analytical skills, with the ability to collect and interpret data to drive decision-making.
  • Knowledge of healthcare quality standards and regulatory requirements (e.g., HEDIS, UDS, CMS).
  • Excellent organizational and project management skills to prioritize tasks and meet deadlines.
  • Strong communication and interpersonal skills, with the ability to engage with staff at all levels.
  • Proficiency in using electronic health record (EHR) systems and quality reporting tools.


Key physical and mental requirements:

  • Ability to lift up to 50 pounds
  • Ability to push or pull heavy objects using up to 50 pounds of force
  • Ability to sit for extended periods of time
  • Ability to stand for extended periods of time
  • Ability to use fine motor skills to operate office equipment and/or machinery
  • Ability to receive and comprehend instructions verbally and/or in writing
  • Ability to use logical reasoning for simple and complex problem solving
  • Ability to travel to multiple work sites as needed
