So, what will the Clinical Pharmacist at Terrestris do?

The VA Healthcare System is light‑years ahead of the private sector in terms of pharmacist autonomy and breadth of clinical practice experience. As the Clinical Pharmacist you will be responsible for ensuring the safe, accurate, and effective dispensing and administration of medications, providing expert clinical services, and ensuring compliance with relevant pharmacy regulations and guidelines. This role will involve collaboration with healthcare teams to enhance patient care and optimize therapeutic outcomes in both inpatient and outpatient environments.

What does a typical day look like for the Clinical Pharmacist?

You will:

  • Review and interpret medication orders (verbal and written) and enterthem into the computerized system in alignment with policies and protocols.
  • Evaluate drug regimens for contraindications, interactions(drug–drug, drug–food, drug–disease), allergies, dosing appropriateness, andpatient-specific factors.
  • Support or manage sterile product services, including TPN andchemotherapy compounding.
  • Provide clinicians with expert guidance on pharmacy services,policies, and medication problems.
  • Participate proactively in interdisciplinary rounds; performmedication reconciliation at transitions of care; optimize therapy continuityfor discharged patients.
  • Handle adverse drug reaction and error reporting through facilitysystems; engage in quality improvement and medication-use evaluationinitiatives
  • Review outpatient medication orders, ensuring proper drugselection, dosing, and safety; screen for interactions, formulary compliance,and inappropriate therapy.
  • Manage non-formulary requests and restricted drug approvals;suggest therapeutic alternatives as needed.
  • Provide medication and disease-state management support, includingpharmacokinetic consultations (e.g., vancomycin or aminoglycosides dosing),monitoring, and lab evaluation.
  • Adverse event reporting, Medication Use Evaluations (MUEs), andmanagement of recalls or medication shortages – including identifying suitablealternatives.
  • Perform medication reconciliation at care transitions; counselpatients or caregivers on medication use and adherence.
  • Enter and process prescriptions, including refill requests,partials, renewals, and Choice/Fee-Basis orders.
  • Dispense and check medications—including controlled substances,investigational drugs, complex preparations, and extemporaneous compoundedformulations—in compliance with federal law and internal policies.
  • Oversee the work of pharmacy technicians, ensuring workflowsmoothness, accuracy, and coverage—as well as patient counseling booths andvault operations.
  • Participate in weekend or holiday rotations, operationalcontingency assignments, and assist with staff gaps as needed.

What qualifications do you look for?

You might be the medical professional we’re looking for if you have:

  • Authorization to work in the U.S. permanently, withoutsponsorship.
  • A minimum of a Bachelor’s degree in Pharmacy(B.S. Pharmacy) from an Accreditation Council for Pharmacy Education(ACPE)-accredited College or School of Pharmacy.; Doctor of Pharmacy (PharmD)degree preferred.
  • Full, current and unrestricted license in anyState, Territory, or Commonwealth of the United States or the District ofColumbia); must be licensed for controlled substances, with qualifications topractice in any state.
  • The requisite credentials enabling the grantingof privileges in accordance with the provisions of VHA Handbook 1100.19.
  • Technical proficiency in the skills necessary tofulfill the position’s requirements, including the ability to speak,understand, read and write English fluently.
  • Verifiable evidence of the completion of all ContinuingMedical Education (CME) / Certified Education Unit (CEU) Requirements.
  • Verifiable experience in both inpatient andoutpatient pharmacy settings.
  • Proof of:
    • A negative Tuberculosis Skin Test (TST) orinterferon-gamma release assays (IGRA); a negative chest radiographic reportfor active tuberculosis shall be provided in cases of positive TST or IGRAresults.
    • Immunity to Measles, Mumps, and Rubella (MMR).
    • Completion and passing generic self-studyblood-borne pathogen training.
  • Excellent communication and interpersonal skills.
  • Critical thinking and problem-solving ability.
  • Attention to detail and high level of accuracy.
  • Ability to work collaboratively in a fast-paced clinicalenvironment.
  • Commitment to patient safety and quality improvement.
