This a hybrid role. For the traditional case management portion, the candidate will work under the direction of a case management supervisor, assessing client’s case management needs and goals, completing treatment planning with clients to address their identified needs and goals, providing referrals and links to community services and resources reflective of the treatment plan, as well as monitoring service delivery and evaluating the effectiveness of resources and service provisions.   For the Supportive Housing Specialist portion, the candidate will work under the supervision of a case management supervisor, following a Housing First model with SMI and non-SMI residents. The candidate must be flexible and self-motivated, able to work both independently and be effective with multi agency communication within the Housing First Model supportive apartment complex.

MAJOR RESPONSIBILITIES

Traditional Case Management:

  • Develops individualized and client centered treatment plans by reviewing intake assessments, identified needs and client’s level of functioning. 
  • Assists clients in developing measurable goals and objectives. 
  • Assists the client in developing a plan to achieve and implement the objectives.   
  • Reviews the treatment plan with clients at maximum of 3-month intervals.   
  • Updates and changes treatment plans upon completion of objectives, changes in needs or goals, or lack of progress toward objectives. 
  • Monitors service delivery and adherence to the treatment plan by providers and clients.
  • Documents implementation of plans using appropriate services, location and time interval codes.  Under supervision, records Case Management activities in a manner required by the agency for both clinical and billing purposes. 
  • Maintains electronic case file in a manner prescribed by the agency.
  • Maintain productivity standards as specified by the department.

Supportive Housing Specialist:

  • Adheres to the philosophy of the Housing First model
  • Provides tenant-centered, culturally responsive, flexible, and individualized supports
  • Provides consistent orientation-informal check-ins, per the Housing Frist model for apartment residents as opposed to episodic scheduled client contacts 
  • Builds trust-based relationships with residents using a non-judgmental, client-centered approach
  • Links residents to voluntary wraparound services
  • Coordinates community-based supports such as employment, benefits assistance, and food access
  • Utilizes a harm reduction, motivational interviewing and strength based approach to client interventions
  • Employs harm reduction techniques (supporting reduction of risky behaviors rather than requiring abstinence)
  • Addresses crises (e.g., psychiatric decompensation, conflict, substance use events) in a calm, trauma-informed manner
  • Intervenes early when behavioral concerns or potential lease violations arise
  • Maintains accurate, timely documentation in compliance with agency requirements.
  • Collaborates with emergency services or mobile crisis teams when needed
  • Works closely with multiple agencies and maintains effective communication with onsite community partners  
  • Ensures documentation is completed timely including documenting grant specific outcome measures

EXPERIENCE, EDUCATION, AND CERTIFICATIONS

  • Bachelor’s degree in a health-related field from an accredited four-year college or university
  • Social work licensure at any level, RN, or counselor preferred.  Licensure must be maintained
  • Current valid driver’s license
  • Vehicle required and willingness to transport client’s’ in personal vehicle when needed.
  • Maintains automobile insurance as acceptable by minimum standards, and in addition names UNLV HEALTH MOJAVE COUNSELING as an additional insured/interested party and maintains “business use” on insurance policy during the course of their employment (travel reimbursed at the Federal rate)

KNOWLEDGE, SKILLS, AND ABILITIES

  • Must have a familiarity of the SMI population, which comprises a strong component of the Housing First apartment population, as well as all consumers in our traditional case management program
  • Self-motivated, with a heightened sense of flexibility, able to work independently, able to communicate effectively with a team and also collaborate with multiple agencies
  • Able and willing to climb stairs (two flights) as the apartment building has no elevator. 
  • In accordance with the conditional offer of employment, individuals must pass a drug screen and a background investigation

PHYSICAL REQUIREMENTS

  • Must be physically able to use stairs in locations where elevators are unavailable
  • May include standing, sitting, and/or walking for extended periods
  • May include performing repetitive tasks
  • May include working on a special schedule (i.e., evenings and weekends)
  • May include working with challenging patients and clients
  • May include lifting up to 25 pounds
