Hospice Health Information Coordinator

What You Will Do
Review and manage Hospice medical record documentation, assuring the agency meets Medicare and insurance requirements. Completes data entry of clinical information. Completes scheduling tasks. Completes quality control reviews of documentation and notifies the appropriate personnel of missed or questionable items. Manages the physician order process and coordinates with the clinical manager and the billing coordinator for the timely release of billing claims.
- Enters and maintains accurate client records, focusing on the regulatory-specified time points, including but not limited to admission, transfer, resumption, discharge, NOE management, status, diagnosis, assessment, medication, physician orders, CTI, LCD, and document management.
- Maintains, updates, and follows up with insurance information and authorization in the EMR, identifying barriers that could potentially prevent payment and escalating to the appropriate personnel.
- Ensure visit management tasks are completed, including but not limited to scheduling visits, ensuring visits are completed accurately and timely, following up with clinicians on outstanding and incorrect items, and escalating to the clinical manager when appropriate.
- Ensures the processing of all regulatory paperwork is compliant and found in the medical record, including but not limited to the plan of care, CTI, LCD, Admission agreement, consent, and election addendum, NOE, IDG, Face-to-Face.
- Provide excellent customer service when talking with staff, clients, families, physicians, or others.
- Assists clinical manager and administrator in special tasks and projects for operational management as requested.
- Travel as needed to off-site facilities, physician practices or other locations as needed/requested.
Qualifications
- Two (2) or more years of experience working with Home Health and/or Hospice Medicare regulatory requirements.
- Experience working with MatrixCare Electronic Medical Record system strongly preferred.
- Has excellent observation, good judgment, and communication skills.
- Two (2) years of post-high school preferred.
- Completion of CEHRS and COC Certification or obtained within one (1) year of completing probationary period.
- Experience in using Microsoft Office Suite programs required.
- Must possess a valid Ohio driver’s license and acceptable driving record. Must be insurable and meet McGregor’s carrier underwriting standards.
- Must have reliable transportation.
