PACE KC is seeking a dedicated and mission driven Enrollment Coordinator to support the PACE KC team in providing the highest quality, all-inclusive care to people 55 and older in Jackson County, Missouri. The Enrollment Coordinator assists the Outreach and Enrollment team in following up with potential participants, coordinating schedules, receiving and returning phone calls, conducting phone screenings to determine possibly qualified candidates, and assists with documentation, community events, and other duties as necessary to facilitate enrollment. The Enrollment Coordinator plays a critical role in our Outreach and Enrollment team, helping to contact prospective participants for the program. 

Primary Responsibilities: 

  • Follows up on leads/prospects from the Enrollment Specialists and various referral sources 
  • Processes documents, confirms eligibility, works with physician offices/facilities and families to successfully enroll the prospect 
  • Conducts phone screenings with new prospects to determine eligibility 
  • Schedules in-home assessments and day center visits for prospects 
  • Supports Outreach and Enrollment Team with enrollment process for new participants 
  • Supports Outreach and Enrollment Team with coordinating community outreach activities including presentations, exhibits, health fairs, and contact with community groups 
  • Facilitates center tours when needed 
  • Maintains all documentation and eligibility standards of PACE program regulations 
  • Other duties as assigned by the Director of Outreach and Enrollment 

Education/Qualification Requirements: 

  • High School diploma or equivalent education required
  • Bilingual (Spanish/English) Highly preferred 
  • Excellent verbal communication skills and professional phone skills  
  • Exhibits caring attitude toward participants and families  
  • Excellent interpersonal and customer service skills  
  • Attention to detail and ability to complete assigned tasks  
  • Proficient with Microsoft Office Suite, web applications, and mobile apps  
  • Capacity to maintain contact and engage with prospective patients/families/key stakeholders after initial interaction, ensuring progress to completion.
