ROLE OVERVIEW and PURPOSE:
Neighborhood Healthcare’s Quality Team is seeking a volunteer to support our Healthy STAR Program, which focuses on outreach for patients with uncontrolled diabetes and elevated blood pressure. This opportunity is ideal for someone interested in healthcare, public health, or patient care coordination. Please note, this opportunity is unpaid.
RESPONSIBILITIES:
Shadow and learn from our El Cajon Care Coordinator to gain insight into patient care and outreach processes
Assist with patient outreach calls, focusing on individuals with uncontrolled diabetes or elevated blood pressure
Schedule patient appointments and document follow-up needs in the system
Support in managing and closing open tasks (over 80 outstanding outreach items) to improve patient care outcomes
Participate in 1:1 training with the Quality Team Lead to develop skills in healthcare outreach and patient communication
QUALIFICATIONS:
- Excellent interpersonal and communication skills
- Ability to maintain confidentiality and handle sensitive information appropriately
- Preferred: Bilingual in Spanish/English
COMMITMENT:
- Minimum of 2 days per week, with availability between Monday through Friday
- Volunteers must commit to serving for at least 2 months
