Position Summary:

The Community Living Supervisor provides supervision of employees and program services for individuals with disabilities living in the community.  This position oversees the implementation of service plans including medical care and behavior supports, and ensures individuals’ outcomes and objectives are being met.  Ensures supports provided meet organizational guidelines, funder requirements and accreditation standards.

Essential Functions:

  • Supervises program activities including reviewing plans and goals with employees; developing daily schedule of activities for individuals and ensuring follow-through; communicating changes in individuals’ health status or behavior to support team; and ensuring that homes provide a safe and healthy living environment for individuals supported. Reviews all organizational communications promptly.
  • Supervises staff including interviewing, hiring and training new employees. Oversees onboarding activities including on-the-job training; supervises employee performance and provides performance coaching and feedback as needed; and completes performance evaluations.  Ensures employees are compliant with all documentation and training requirements.
  • Ensures that homes operate in accordance with established staffing ratios and overtime guidelines. Partners with workforce team for scheduling and/or coordinates scheduling in non-ISL locations.
  • Supervises the maintenance of all required documentation for individuals served. Oversees and provides documentation for individuals supported and ensures compliance with all organizational, funder and accreditation requirements. 
  • Organizes and reports data related to individual plans including monthly documentation; and attends ISP meetings and service coordinator visits and provides input into the planning process.
  • Oversees house accounts and individuals’ personal spending accounts to ensure accurate records are maintained and proper procedures are followed. Audits household spending regularly.
  • Attends nursing visits and provides medication oversight for individuals served. Reviews all medications and medical administration records.  Reports and documents any charting or medication errors according to established protocols.  Attends key doctor’s appointments and provides training on adaptive equipment. Partners with nurse to ensure appropriate medical supports.
  • Attends meetings, in-services and opportunities for ongoing education. Facilitates and participates in employee meetings and reviews any issues or training needs.
  • Works as direct care staff as needed.

Qualifications:

  • Must have high school diploma or equivalent. Completion of college-level coursework in the human services field preferred.
  • At least one year of experience in a direct care position supporting individuals with disabilities.
  • Previous supervisory experience preferred.
  • Must have knowledge of Microsoft Office suite and be able to work with a variety of computer systems.

Additional Requirements:

  • This position may work day, evening, night and/or weekend hours depending on the needs of the organization. Provides 24-hour on-call support to individuals and employees, with rotating weekend on-call duties.
  • This position requires driving personal and/or company vehicle. Transports individuals to appointments, community activities, job sites and other activities/programs. 
  • Must have reliable vehicle and maintain current driver’s license and proof of insurance.
  • Must complete and maintain all required trainings/certifications.
  • Must maintain confidential information in accordance with HIPAA regulations.
