Description

The Case Manager for the Brighter Days Program plays a vital role in helping individuals at risk of opioid use disorder (OUD) access the care and support necessary for long-term recovery. This position provides trauma-informed, culturally responsive case management and resource navigation services in partnership with community agencies, treatment providers, and internal SGA teams. The Case Manager will support program participants through assessments, service planning, linkage to recovery supports, and coordination of services that promote stability and wellness. Services may be provided in office, community, or outreach settings.



MAJOR DUTIES AND RESPONSIBILITIES

Key Responsibilities:

• Conduct intake assessments to determine client needs across behavioral health, medical, housing, employment, and social support domains.

• Develop and monitor individualized service plans, including goal setting and progress tracking.

• Link clients to recovery services including MOUD (medications for opioid use disorder), inpatient/outpatient programs, 12-step meetings, and harm reduction supports.

• Facilitate warm hand-offs to providers and partners for housing, job training, education, and healthcare.

• Assist clients in accessing transportation, health insurance, and public benefits.

• Maintain a current knowledge of community resources and develop strong partnerships with local providers (e.g., Gateway Foundation, Salvation Army, NIRCO).

• Document all client contacts and case notes following funder and agency timelines.

• Track and report client progress and program outcomes using internal data systems.

• Identify and engage clients through outreach and referral sources, including schools, courts, shelters, and faith-based organizations.

• Participate in reflective supervision, case staffing, and professional development activities.

• Comply with HIPAA and always uphold strict client confidentiality.


Requirements

QUALIFICATIONS FOR THE JOB

• Bachelor’s degree in social work, Human Services, Counseling, or related field required.

• LSW, CADC, or other behavioral health credential preferred.


Experience:

• At least 2 years of experience in case management, behavioral health, substance use treatment or related human services.

• Experience providing trauma-informed services and working with individuals affected by substance use is highly preferred.

Skills and Competencies:

• Strong understanding of behavioral health and recovery services.

• Ability to engage diverse clients using empathy, motivational interviewing, and cultural humility.

• Excellent written, verbal, and interpersonal communication skills.

• Proficient in Microsoft Office and case documentation systems.

• Strong time management, problem-solving, and organizational skills.

Other Requirements:

• Must have reliable transportation, a valid driver’s license, and proof of insurance.

• Willingness to work occasional evenings and weekends.

• Bilingual (English/Spanish) preferred.

PHYSICAL REQUIREMENTS

• Ability to travel to and from client homes, community settings, and office locations

• Occasionally lift up to 25 lbs. for outreach events and group facilitation

• May require climbing stairs or walking extended distances during home visits
