You'll be responsible for coordinating direct care scheduling, monitoring tracking systems, and providing clerical support to the assigned team. Schedule: Monday-Friday 8:30 am – 5:00 pm

Essential Job Functions/Responsibilities:
- Coordinate the SOC visit by interacting with the patient or their representative.
- Proactively plan and assign patients to clinical staff.
- Perform visual checks of forms for discrepancies and process discharge documentation in a timely manner.
- Maintain accurate logs and distribute reports to appropriate staff.
- Update weekly schedules for interdisciplinary team staff and notify the Clinical Team Manager of discrepancies.
- Verify visits provided against visits ordered and scheduled.
- Receive and relay messages for assigned interdisciplinary team staff.
- Prepare schedules for weekend and on-call coverage and order medical supplies as needed.
- Participate in on-call rotation for weekend coverage and answer phones after hours.
- Support the Agency's customer service program by maintaining a responsive and knowledgeable approach to customer contacts.
- Perform other activities as assigned by the Clinical Team Manager.
Position Qualifications:
- High school graduate; college degree preferred.
- Proficiency in Microsoft Office.
- Exceptional organizational and multitasking skills.
- Excellent telephone and customer service skills.
- Strong work ethic and time management skills.
- Demonstrates patience, flexibility, and a cooperative attitude.
- Comfortable participating in weekend and telephone on-call rotation.
