How You’ll Make a Difference (Essential Functions)

The Homecare Manager, under the supervision of the Regional Director, provides leadership, guidance, and direct supervision for all aspects of daily operations for the Homecare team, to include management of staff, ensure staffing to meet patient needs and coordination of care. This position will ensure high quality and safe care delivery while promoting a positive and innovative culture through coaching, facilitation, decision-making, problem-solving, census building and regulatory expertise. Responsibilities include coordinating direct and indirect patient care using interdisciplinary team (IDT) approach within a complex and changing environment. This position maintains accountability for quality outcomes, financial performance, and the growth and development of the team.

 

Key Responsibilities 

  • Daily Operations Management  
    • Ensure appropriate staffing levels and allocation for services based on census/family needs.
    • Ensure appropriate assignment of new admissions/transfers to ensure consistency in care group caseloads and meeting patient/family needs through use of stablished business models and in collaboration with other leadership supporting the region.
    • Provide oversight and guidance on the management of complex/acute patient cases.
    • Assist clinical staff in the need to facilitate coordination of care. (e.g., partner coordination, appropriate level-of-care utilization).
    • In collaboration with other members of leadership, monitor team performance ensuring continuous quality improvement and high levels of patient, family, and partner satisfaction while reinforcing established clinical, quality, and regulatory expectations.
    • Ensure clinical staff provide quality, consistent, efficient, and cost-effective hospice services while maintaining regulatory compliance and practicing within the scope of their discipline.
    • Direct the clinical practice of the home care team and standardizes processes, with other home care leaders, to ensure consistency across care delivery.
    • Proactively work with team members to address patient/family concerns and safety issues (patient or staff).
    • Collaborate with Volunteer Services to ensure that home care staff are adequately trained to effectively utilize volunteers in supporting patient needs.
    • Provide leadership support and guidance to 24/7 operations in on-call capacity as needed.
  • Management of Staff and Employee Engagement
    • Participate in recruitment process, make hiring decisions, and develop training plans for new team members.
    • Assess staff performance and provide feedback, coaching, and/or correction.
    • Complete performance evaluations per agency protocols.
    • Coach staff to work within established job description guidelines and maintain boundaries.
    • In conjunction with the Clinical Education and Quality/Compliance team, identifies educational needs of team and facilitates their development and knowledge into practice.
    • Work with preceptors to evaluate readiness of new staff to work independently.
    • Promote culture of learning, recognize concerns, resolve fears, and motivate the team for engagement.
    • Assist the Clinical Education team to ensure all team members complete all required competencies timely and successfully.
    • In conjunction with compliance HNS team, ensures regulatory visits are completed in accordance with regulatory requirements and Agrace policy.
    • Develop and direct engagement plans to ensure high levels of team engagement and retention in collaboration with other leaders and staff
    • Engages and empowers direct clinical care staff in improving care delivery and systems.
  • Financial Performance
    • Monitors team financial performance against established standards; identifies the underlying cause of financial performance variances and takes corrective action as needed.
    • Review staff productivity and create action-plans for addressing issues and needs.
    • Coach staff to understand financial implications of care and ensure following of formularies (pharmacy and durable medical equipment) and policies related to care delivery.
    • Interpret operational indicators to detect census changes and fluctuations in volume that could impact staffing levels, revenues, or expenses.
  • Other Duties
    • Participate in internal work groups, meetings, and committees relative to position responsibilities.
    • Develops and strengthens partnerships within the community in an effort to provide education, deliver high quality services and continuously focus on increasing census and meeting community need (ie hospitals, clinics, health care facilities, schools and vendors).
    • Effectively teaches staff to create partnerships, recognize business development opportunities and actively participates in outreach activities that lead to agency referrals.

 

You Bring (Qualifications)

  • Bachelor’s degree with clinical background, preferred.
  • Current unrestricted Wisconsin RN license or SW license, preferred
  • 5 - 7 years clinical leadership and/or a healthcare administrative role, required; hospice environment, preferred
  • Five years progressively responsible supervisory experience
  • Valid driver’s license, auto insurance, and access to a reliable vehicle
  • Remote management experience preferred
  • Must be detail oriented, with a demonstrated ability to work independently and organize a demanding workload with accuracy
  • Must have excellent verbal and written English communication skills
  • Knowledge of and experience with health care delivery systems, policies, accreditation standards, laws, regulations, and reimbursement practices required; hospice-specific knowledge and experience, preferred
  • Experience in hiring, leading, educating and/or coaching an interdisciplinary healthcare team in the provision of holistic care
  • Experience conceiving, designing, developing, executing, and maintaining project plans and administrative policies and procedures for compliance with regulations
  • Ability to communicate financial/business/technical/clinical information to all levels within and outside the organization including group presentation skills
  • Intermediate Microsoft Office Suite skills
  • Experience using electronic health record software
  • Ability to use proper body mechanics and standard precautions
