Now, we’re looking for a Clinical Nurse Care Coordinator (CNCC) to join our home health division and be a vital link in ensuring seamless, high-quality care for our patients.

What You’ll Do
As our CNCC, you’ll be the bridge that keeps our patients’ care plans running smoothly. Your days will include:
Coordinating Care: Work closely with physician offices, RN Case Managers, and other disciplines to ensure patients receive timely and accurate care.
Clinical Triage: Review labs, follow up on hospitalizations, manage transitions of care, and facilitate additional skilled services when needed.
Documentation & Compliance: Maintain accurate, timely records in Kinnser (WellSky) and help keep our quality assurance standards high.
Medical Supply Coordination: Order and track DME and supplies in line with patient needs.
Interdisciplinary Collaboration: Serve as the go-to person for clinical communication across the care team.
What We’re Looking For
Texas-licensed LVN or RN in good standing
1+ years clinical experience (home health or care coordination preferred)
Experience with Kinnser/WellSky strongly preferred
Strong clinical judgment, organization, and communication skills
Proficiency in Microsoft Office and comfort with technology
